Objective:This study evaluated the safety and feasibility of spontaneous breathing anesthesia video-assisted thoracoscopic surgery(VATS)for Lung bullae surgery and it has potential advantages over traditional tracheal intubation anesthesia.Method:Retrospective analysis of the clinical data of 216 patients who underwent VATS bullous resection in our hospital from October 2016 to December 2018,24 of them were admitted to patients with spontaneous breathing anesthesia and the age,sex,smoking,Body weight index,comorbidities were compared in 24 patients who underwent conventional tracheal intubation for general anesthesia.Compare the preoperative induction catheter anesthesia time,postoperative recovery time,operation time,postoperative hospital stay days,postoperative chest drainage tube placement days,blood loss,pain grade score,postoperative pulmonary complications(pulmonary leak,atelectasis,subcutaneous emphysema,lung infection)and postoperative airway complications(sound hoarseness,sore throat,oropharyngeal foreign body sensation)and other indicators.Result;There were no deaths in the two groups,and there was no intraoperative anesthesia in the SA group.In the SA group,the time of induction catheter anesthesia(15.04±5.64ns 25.08±11.23 min,P<0.001),recovery time(22.87±10.61 ns35.79±13.02 min,P<0.001),postoperative pain score 24 hours(5.04±2.87 n s6.67±2.50,P=0.042),48 hours(4.07±3.07 n s 5.91±2.46,P=0.035),72 hours(2.58±2.55 ns 3.92±1.67,P=0.037),postoperative airway complications(1 case in SA group,4%;8 cases in GA group,33%,P=0.01)was significantly better than the GA group.There was no significant difference in the amount of blood loss(18.33±9.54ns 20.67±6.26 ml,P=0.322),operation time(39.2±12.5 ns 37.3±11.4 min,P=0.663),postoperative chest drainage tube placement days(4.87±1.15 n s 5.04±1.33 d,P=0.646),postoperative hospital stay(5.46±1.47 n s 6.13±1.82 d,P=0.171),postoperative pulmonary complications(there were 4 cases in the SA group,17%;6cases in the GA group,25%;P=0.477)between the two groups during the operation.Conclusion:Spontaneous breathing anesthesia VATS is safe and feasible in the treatment of lung bullae surgery.Compared with the GA group,the SA group had obvious advantages in preoperative induction of anesthesia time,postoperative recovery time,postoperative pain perception,and postoperative airway complications.
