Objectives:1.To compile the questionnaire of obstetrics nurse 's insulin therapy knowledge by Delphi method.2.Investigating the status and training demands of Nanchong tertiary hospital obstetricsnurses' insulin therapy knowledge.3.Exploring influencing factors of Nanchong tertiary hospitals obstetrics nurses' insulin therapy knowledge.4.Providing the solutions and countermeasures combined with the actual situation to improve the status of insulin therapy knowledge of Nanchong tertiary hospitals obstetrics nurses.methods:This study is a cross-sectional survey.Based on literature reviews,the preliminary questionnaire of obstetric nurses' insulin treatment knowledge was compiled through 3 rounds group discussions.After two rounds of Delphi experts consulting and verifying the reliability and validityby pre-survey,this questionnaire was finally finished.A cluster sampling method was used to conduct the questionnaire survey on a total of 263 nurses in nine tertiary hospitals obstetrics departments in Nanchong City obstetrics.Microsoft Excel was used to input data,and SPSS 26.0 was used for statistical analysis.Results:1.After two rounds of expert consultation,the questionnaire was determined to have 3 first dimensions and 63 seconddimensions.The expert coordination coefficients were 0.123 and 0.322 and were statistically significant,P<0.05.The two rounds of consultation experts' positive coefficient of 100%and authority coefficients are all above 0.7.Through the pre-survey,the Spearman-Brown value of the questionnaire is 0.945,indicating that the questionnaire has a half-reliability.The Kudar-Richardson Formula 20(KR-20)value is 0.973,which also indicates that the questionnaire has good reliability.The content validity method is used to analyze the validity of the questionnaire,both I-CVI and S-CVI were higher than 0.9,indicating good validity.2.Through cluster sampling,a questionnaire survey was conducted on 263 nurses in the obstetrics department of 9 tertiary hospitals in Nanchong.A total of 263 questionnaires were distributed and recovered.The results are shown below::?The total score of the knowledge about diabetes and gestational diabetes is 16 points.In this study,the dimension score is 10.061±4.529,the number of passers in this dimension is 146,accounting for 55.51%and shows that the nurses have poor knowledge in this dimension.Three items with lower scores are:gestational diabetes can increase the incidence of postpartum depression(0.502±0.501),ideal blood glucose control goals for type 2 diabetes(0.437±0.497),gestational diabetes blood sugar target(0.487±0.501)Single factor analysis shows:education,job titles,diabetes specialist nurse,whether they have studied insulin standard injection courses and training,whether they have provided pregnancy diabetes insulin injection and management advice to patients,whether they are proficient in relevant knowledge are influencing factor that affects the relevant knowledge in the dimension,the results are statistically significant;multiple stepwise regression analysis prompts:education and whether to provide pregnancy diabetes insulin injections and management advice to pregnant diabetes patients are main influencing factors that affect the score of this dimension.?Insulin-related knowledge has a total score of 11 points.In this study,the obstetric nurses scored(5.517±3.416),and the passing number was 102,accounting for only 38.78%,suggesting that the obstetric nurses did not have good knowledge of insulin-related knowledge.Three items with lower scores are:effective time after subcutaneous injection of fast-acting insulin(0.319±0.467),effective time after subcutaneous injection of short-acting insulin(0.327±0.470),start time and duration of long-acting subcutaneous injection(0.312±0.464).Single factor analysis tips:different hospital levels,education,diabetes specialist nurse,the frequency of insulin standard injection training courses carried out by the department,whether they have studied the insulin standard injection training courses and training,whether they have provided pregnant diabetes insulin injections and management advice to patients are factors that affects the scores of insulin-related knowledge of obstetric nurses;Multiple stepwise regression analysis shows:education,diabetes specialist nurse,and whether they have provided pregnant diabetes insulin injections and management advice to patients are the main factor in this dimension.?Insulin injection related knowledge has a total score of 36 points.In this study,the obstetric nurses scored(22.490±10.246),and the number of passers was 151,accounting for only 57.41%.Three items with lower scores are:when it is necessary to slow down the absorption rate of insulin,you can choose the buttocks for injection(0.426±0.495),the insulin special syringe plug can be pulled out without staying under the skin for 10s(0.403±0.491),early pregnancy does not need to change the insulin injection site or injection technique(0.411±0.493).Single factor analysis shows:education(P<0.01),diabetes specialist nurse(P<0.05),the frequency of the department conducted the insulin standard injection training(P<0.01),whether nurses has studied insulin-related courses and training(P<0.05)Whether applying the knowledge in clinical(P<0.01)and whether the knowledge is proficient(P<0.05)is a factor that affects the score of insulin injections knowledge of nurses in obstetrics and gynecology.Multiple stepwise regression analysis prompts:the level of education of obstetrics and gynecology nurses and whether they have applied knowledge in clinical are important factors in this dimension.Single factor analysis shows:educational,diabetes specialist nurse,the frequency of insulin standard injection training courses carried out by the department,whether they have studied the insulin standard injection training courses and training,whether they have provided pregnant diabetes insulin injections and management advice to patients,and whether obstetrics nurses are proficient in knowledge are factors that affects the knowledge of obstetric nurses' insulin standard injection.Multiple stepwise regression analysis prompts:education and whether they have provided pregnant diabetes insulin injections and management advice to patients are important factors in this dimension.3.The total score of the insulin treatment knowledge of obstetric nurses in the tertiary hospital of Nanchong City was 63 points.Nurses got only(38.068±16.285),and the number of passers was 143,accounting for 54.37%,suggesting that the obstetric nurses of tertiary hospital in Nanchong City are lack of the knowledge of insulin therapy.4.In the study of the demand for insulin training by obstetric nurses in the tertiary hospitals of Nanchong City,nearly 98%of obstetric nurses believed that they needed knowledge about insulin therapy.Conclusions:1.In this study,Delphi expert consultation method was used to compile a questionnaire on the knowledge of insulin treatment of obstetric nurses in the tertiary hospitals of Nanchong City,and the reliability and validity were good.2.The scores of three dimensions and the total score of insulin treatment knowledge of obstetric nurses in the tertiary hospitals of Nanchong City are low,suggesting that the three dimensions and the overall mastery are poor,and the situation is not optimistic.3.Through this study,it was found that:education,diabetes specialist nurse,whether they had studied insulin standard injection courses and training,whether they have studied the insulin standard injection training courses,and training,whether obstetrics nurses are proficient in knowledge are factors that affects all the dimensions and overall scores.In addition,the frequency of the department's insulin standard injection training courses are the influencing factors of insulin-related knowledge,insulin-standard injections,and the total score of knowledge.The job title influences the scores of knowledge related to obstetric nurses' diabetes and hyperglycemia during pregnancy;the hospital grade influences the score of knowledge related to insulin.4.The obstetric nurses of the tertiary hospitals in Nanchong City have participated in less training related to diabetes or insulin therapy and have a higher demand for related knowledge.
