Objective:Collected the case data of male patients aged 9 to 14 years who visited the pediatric surgery clinic of West China Hospital of Sichuan University in 2019-2020.To analyze the effects of overweight and obesity on height development,metabolic level and sexual development in prepubescent and early adolescent male.Methods:1.Case data of patients aged 9 to 14 years who visited the pediatric surgery clinic of West China Hospital of Sichuan University from February 2019 to January 2020 were collected through the outpatient medical record system.Patients were screened according to inclusion and exclusion criteria,and were divided into normal weight group,super recombination group and obesity group according to body mass index(BMI).The effects of overweight and obesity on prepubescent and early adolescent male were analyzed from height development,metabolic,sexual development.The clinical characteristics of overweight and obese children in diet,exercise,self cognition and family cognition were explored by analyzing the questionnaire.2.EpiData 3.1 was applied for statistical treatment of case data.SPSS 17.0 statistical analysis soft was applied for analyzing all the data.And this study used GraphPad Prism 7 to make charts.At first,the data were tested for normality.If the data were in line with the normal distribution,it was expressed as mean ± standard deviation.T-test was used for comparison between two groups,and ANOVA was used for analysis of variance between multiple groups.If the data were not in line with the normal distribution,it was expressed as the median(25percentile-75percentile).Mann-Whitney rank sum test was used for comparison between two groups,and Kruskal-Wallis H rank sum test was used for comparison between multiple groups.The Chi-square test was used to compare the intergroup rates.Logistic regression was used to analyze the possible influencing factors of overweight and obesity in adolescent males.P<0.05 was considered statistically significant.Results:1.In this study,228 patients were included,including 73 in the normal weight group(NC),57 in the overweight group(OW)and 98 in the obese group(OB),with the overweight and obesity rates of 25.00%and 42.98%,respectively.There was no significant difference in age composition among the three groups(P>0.05).?Height development:OW and OB were higher than NC(P<0.01).However,there was no significant difference in height between the OW and OB(P>0.05).?Metabolic:Uric acid(URIC):OW and OB were higher than NC(P<0.001).However,there was no significant difference between the OW and OB(P>0.05).Triglycerides(TG):OW was higher than NC(P<0.01).However,there was no significant difference between the OB and NC(P>0.05).And there was no significant difference between the OB and OW(P>0.05).High density lipoprotein cholesterol(HDL-C):OW and OB were lower than NC(P<0.001).However,there was no significant difference between the OW and OB(P>0.05).Low density lipoprotein cholesterol(LDL-C):OW was higher than NC(P<0.05).However,there was no significant difference between the NC and OB(P>0.05).And there was no significant difference between the OW and OB(P>0.05).Glucose(GLU),Cholesterol(CHOL)and Creatinine(CREA):There was no significant difference between the NC,OW and OB(P>0.05).?Sex hormone:Luteinizing hormone(LH):OB was lower than NC(P<0.001).However,there was no significant difference between the OW and NC(P>0.05).And there was no significant difference between the OB and OW(P>0.05).Prolactin(PRL):OB was higher than NC and OW(P<0.05).However,there was no significant difference between the NC and OW(P>0.05).Estradiol(E2):OB was higher than NC(P<0.001).However,there was no significant difference between the OW and NC(P>0.05).And there was no significant difference between the OB and OW(P>0.05).Sex hormone binding globulin(SHBG):OW and OB were lower than NC(P<0.001).And OB were lower than OW(P<0.05).Progestin(P):OB was higher than NC and OW(P<0.05).However,there was no significant difference between the NC and OW(P>0.05).Free testosterone(FT):OW and OB were higher than NC(P<0.05).However,there was no significant difference between the OB and OW(P>0.05).Follicle-stimulating hormone(FSH)and Dehydroepiandrosterone sulfate(DHEA-S):There was no significant difference between the NC,OW and OB(P>0.05).? Testis volume:OW was bigger than NC and OB(P<0.05).However,there was no significant difference between the NC and OB(P>0.05).2.Onset of puberty:There were 27 cases of NC,28 cases of OW and 29 cases of OB with puberty onset,their median ages were 12.0,12.0 and 12.5,and there was no significant difference between the NC,OW and OB(P>0.05).Accounting for 36.99%,49.12%and 29.59%of the total number of people in each group,and OW was higher than OB(P<0.05).3.Questionnaire survey:A total of 87 questionnaires were collected,3 incomplete questionnaires were excluded,and 84 questionnaires were analyzed and summarized.There were 26 cases in NC,21 cases in OW and 37 cases in OB,accounting for 31%,25%and 44%respectively.Preference for meat was a risk factor for overweight and obesity in adolescents(OR=5.709,95%CI=1.179-27.633).However natural birth was a protective factor for overweight and obesity in adolescents(OR=0.086,95%CI=0.012-0.629).Conclusions:1.In prepuberty and early adolescence,overweight and obese men have a higher height development advantage than normal weight men,and the overweight group is superior to the obese group.2.In early and early adolescence,overweight and obesity can lead to abnormal purine and lipid metabolism in adolescent males.3.Obesity can interfere with the sexual development of adolescent male from pituitary and gonad,while overweight mainly interferes with the functional state of testosterone.4.The endocrine system disorder caused by overweight and obesity has been affecting the development of male testicles since early adolescence.5.Overweight and obesity do not lead to earlier onset of puberty in males,but the rate of OW into puberty is higher than OB.6.Preference for meat is a risk factor for overweight and obesity in adolescents,and natural birth is a protective factor for overweight and obesity in adolescents.
