Background:Gastric cancer(GC)has always been a persistent disease that endangers human health.In 2018,gastric cancer caused a total of 1 million new cases and approximately 783,000 deaths globally.Among all cancers,the incidence of gastric cancer ranks fifth in the world,and the mortality rate ranks third.In China,there were more than 450,000 new cases of gastric cancer in 2018,and more than 390,000 new cases of gastric cancer-related deaths,accounting for 13.6%of the total number of cancer deaths in the country.After continuous exploration of gastric cancer diagnosis and treatment,the mortality rate of gastric cancer in most countries in the world including China has declined,but the characteristics of high incidence and high mortality of gastric cancer in my country have not been fundamentally changed.At present,the treatment of gastric cancer has entered a stage of the combination of various comprehensive treatment measures including surgery,endoscopic treatment,radiotherapy and chemotherapy,but the status of patients with a generally poor prognosis has not been significantly improved.At present,the treatment of gastric cancer has entered a stage of the combination of various comprehensive treatment measures including surgery,endoscopic treatment,radiotherapy and chemotherapy,but the prognosis of patients is generally poor.The status quo has not been significantly improved.Improving the survival rate of patients with gastric cancer has always been a key area of concern in the treatment of gastric cancer in the medical community.The onset,development and prognosis of gastric cancer are affected by various factors such as geographical environment,Helicobacter pylori(HP)infection,chronic diseases,genetics and genes,and their own physiological status.More understanding of these cancer risk factors may help Used in the treatment of gastric cancer.Although some risk factors have been confirmed by research to affect the prognosis of gastric cancer,gastric cancer itself is complex and changeable,and its epidemiology and clinicopathological characteristics still have a large degree of uncertainty.Objective:Retrospectively analyze the clinical medical records of gastric cancer patients undergoing radical surgery for gastric cancer,clarify the correlation between clinical-pathological factors and prognosis of gastric cancer after surgery,and provide a theoretical basis for comprehensive treatment of gastric cancer patients after surgery.Methods:Retrospective analysis of the medical records of 597 patients who underwent radical gastrectomy in Shandong Provincial Hospital from 2008 to 2014,focusing on the gender,age,perioperative blood transfusion,surgical method,TNM stage,tumor invasion depth,lymph node metastasis,and tumor Factors such as size,site of disease,Lauren classification,degree of differentiation,etc.,Kaplan-Meier method derived survival rate and survival curve,correlation verification using the long-rank test,introducing COX model for multi-factor analysis.Results:A total of 597 patients with gastric cancer survived to the end of the follow-up period.A total of 292 patients survived.The calculated 1-year,3-year,and 5-year survival rates were 92.5%,68.2%,and 48.9%.Univariate analysis showed that the patient's gender(P>0.05),age(P>0.05),surgical method(P>0.05),degree of differentiation(P>0.05),perioperative blood transfusion(P>0.05)and gastric cancer patients Postoperative prognosis is irrelevant;TNM stage(P<0.05),depth of invasion(P<0.05),lymph node metastasis(P<0.05),tumor size(P<0.05),site of disease(P<0.05),Lauren classification(P<0.05)is related to the postoperative prognosis of gastric cancer patients.Multivariate analysis showed:TNM stage(P<0.05),depth of invasion(P<0.05),lymph node metastasis(P<0.05),tumor size(P<0.05),site of disease(P<0.05)can be used as postoperative gastric cancer patients Independent influencing factors of prognosis;the pathological types of tumors according to Lauren classification(P>0.05)cannot be used as independent influencing factors of postoperative prognosis of gastric cancer patients.Conclusion:(1)TNM stage,lymph node metastasis,depth of invasion,tumor size,tumor site,Lauren classification are associated with the postoperative prognosis of gastric cancer patients(2)TNM stage,lymph node metastasis,depth of invasion,tumor size,and location of the disease are independent factors affecting the prognosis of gastric cancer patients after surgery.
