Cervical Sagittal Parameters Of The Patients With Cervical Osseous Hyperplasia Disease:A Comparative Cross-sectional Study

Background:The spinal sagittal parameters are known closely related to spinal degeneration diseases.However,there are few reports focused on the comparison of cervical sagittal parameters among patients with COHD and healthy controls.Methods:A comparative cross-sectional study of data from 58 patients with OPLL(22 females/36males),76 patients with PVO(31 females/45males)and 80 adults(54 females/26 males)with non-specific neck pain in whom basic information[1],disease classification and imaging materials were obtained.The Cervical Sagittal Vertebral Alignment(cSVA)mm,Cobb angle C0-C2(c0-2 cobb)°,Cobb angle C2-C7(c2-7 cobb)°,T1 slope(T1S)°,Neck Tilt(NT)°,and Thoracic Inlet Angle(TIA)°measured from the neutral cervical lateral x-ray were assessed.Data analysis was performed using statistical description,independent sample T test and ANOVA.Results:There was significant difference in age among OPLL(57.67),PVO(53.41)and Control group(52.48)(p=0.001).While no significant difference was found between PVO and Control group(p>0.05).We found that T1 slope angle(26.83±6.63),c2-7cobb(11.08±8.6782)of PVO were significantly smaller than those of Control group,TIS(29.61 9±7.3001)and c2-7cobb(14.76 ± 9.54)(P=0.014?p=0.013 respectively).While cSVA of PVO group(19.69 ± 12.96)was higher than Control group(15.61±8.97)(P=0.025).No significant difference was found between genders and lesion segments.Among PVO group,myelopathy(M)patients' T1S(28.63±6.60),c2-7cobb(13.41 ±9.51)are significantly larger than those of radiculopathy(R)group,T1S(24.82±6.14)and c2-7cobb(8.50±6.90)(P=0.011?P=0.012 respectively).However NT(53.25± 5.87)and c0-2cobb(20.44± 9.32)are smaller than R group,NT(57.57±6.61)and c0-2cobb(26.25±8.91)(P=0.003?P=0.007 respectively).Among OPLL patients,the course of OPLL patients with cervical myelopathy(M)(37.52)is much longer than that with radiculopathy(R)(9.78),and c2-7cobb(18.90 ±9.22)is larger than R group(12.99±7.93)(p=0.041).There is no significant difference between different lesion segments of OPLL group.Conclusion:There was significant difference of cervical sagittal parameters between PVO and Control patients.The same was that in M and R type of PVO and OPLL patients.The occurrence of PVO may closely related to long-term and long-time keeping head down.And age may be an independent influencing factor for development of OPLL.Further,with the progress of the disease,R type OPLL may exacerbate to M type,which demonstrates the necessity of early intervention in disease.
