[Objective] The aim of this study was to explore the correlations between the different parameters of the cervical sagittal balance in radiological,related to disc-degenerative diseases and try cervical sagittal balance parameters to guide the feasibility of sagittal balance after reconstruction.[Methods] Chose the healthy volunteers of 274 whose neck radiologicals were obtained.The parameters measured in the neck radiologicals were: Cobb's angle(C2–7)?T1 slope(T1S)?thoracic inlet angle(TIA)and neck tilt(NT).The correlation between the various parameters was analyzed using the Pearson correlation coefficient,establish the regression equation of straight line.We conducted a retrospective review of the radiological of 158 inpatients with disc-degenerative diseases of the cervical spine;the images were obtained between Jan 2010 and Dec 2016 at our institute,measuring the same cervical sagittal balance parameters,and compared with healthy volunteers,related to disc-degenerative diseases and analyzed of preoperative and postoperative patients with sagittal balance parameters change.According to JOA(Japanese Orthopedic Association)score calculate JOA improvement;Calculate actual value(deviation angle)according to C2-7COBB theoretical value which obtained through regression equation.Divide every 3°in to groups and compare the difference between the JOA improvement.[Results] The average demographic data of healthy volunteers:T1S: 26.38+7.49°;C2-7 COBB : 18.71+9.77°;NT : 47.59+6.63°;TIA : 73.98+10.03°;T1S and C2-7COBB obvious positive correlation(0.569,p < 0.05),establish the regression equation of straight line: C2-7COBB=0.742T1S-0.866.The average demographic data of disc-degenerative diseases: T1S: 24.01+7.81°;C2-7 COBB : 14.02+9.66°;NT : 47.94+6.67°;TIA : 71.95+9.69°.The healthy volunteers group has lower T1S(p<0.05)and C2-7COBB(p<0.05)than disc-degenerative diseases.The mean follow-up period was 3-5 months,the cervical JOA increased from 9.67+1.44 to 14.94+1.14(p<0.05),0-3°JOA improvement was 87.00+6.09%,3-6°JOA improvement was 68.16+11.00%,more 6°JOA improvement was 57.67+10.63%.The JOA improvement has statistical significant difference with deviation angle in disc-degenerative diseases(p<0.05).[Conclusions] The healthy volunteers group showed moderate positive correlation between T1 S and C2-7COBB,we can establish the regression equation of straight line.The disc-degenerative diseases has lower T1 S and C2-7COBB than healthy volunteers group.Lower T1 S and C2-7COBB may cause disc-degenerative diseases.JOA has significant improvement after cervical surgery.Healthy volunteers cervical sagittal balance parameters could establish regression equation,may provide theoretical basis and guide cervical spine surgery for the reconstruction of the cervical spine curvature.
