ObjectivesTo investigate the treatment effect and compliance through family empowerment nursing on child patients with lobar pneumonia who were also conducted electronic bronchoscopy,and to observe the influence of empowering nursing on the identification of empowering nursing,care load,psychological influence and nursing satisfaction of primary caregivers.Methods240 children with lobar pneumonia who needed bronchoscopy and were treated in the pediatric ward of our hospital from February 2018 to June 2019 were collected as research objects.The children were randomly divided into experimental group and control group,with 120 children in each group.In the control group,children were treated with routine pediatric care and bronchoscopy for lobar pneumonia.The experimental group adopted the same method as the control group,and combined with family empowerment nursing in advance.Its aim is to observe and compare multiple clinical treatment indicators,treatment compliance and adverse events after bronchoscopy.Likert Scales were used to compare the total score,self-support dimension and emotional support dimension of primary caregivers.Chinese version load assessment scale was used to measure caregiver's nursing load from three dimensions:physical function,psychological function and social function.Self-rating anxiety scales(SAS)and self-rating depression scales(SDS)were used to assess the psychological state of the primary caregiver.At the same time,a nursing satisfaction questionnaire was developed to evaluate the satisfaction degree of the two main groups of caregivers.All data were analyzed according to SPSS21.0.The measurement data were expressed as mean ± standard deviation(x±s),and the t test was adopted.The counting data were expressed as"n" or percentage,and the ?2 test was adopted,P<0.05,which is statistically significant.Results1.The remission time of cough symptoms in the experimental group and the disappearance time of pulmonary rales sound in the experimental group were 6.8±2.1 d and 6.3±2.1 d,which were lower than that in the control group(8.6±3.I d and 7.5±2.4d respectively).There was no significant difference in fever receding,white blood cell restoration,c-reactive protein recovery time and procalcitonin recovery time.2.The treatment compliance of children in the experimental group was significantly higher than that in the control group in terms of standardized medication,regular examination,reasonable diet,nursing cooperation and other aspects.3.The incidence of dysphoria after bronchoscopy was 15%in the experimental group,which was significantly lower than 32%in the control group(P<0.05).There was no difference in the incidence of complications such as fever,cough aggravation,pharyngeal pain,laryngeal edema and blood in sputum between the two groups(P>0.05).No serious complications such as dyspnea and hypoxemia occurred in the two groups.4.The total score,self-support dimension and emotional support dimension of the primary caregivers in the experimental group were 4.0±0.6,4.4±0.6 and 4.3±0.7 respectively,which was significantly higher than that of the control group3.6±0.5,3.6 ± 0.4 and 3.9 ± 0.9(P<0.05)?5.The scores of the primary caregivers in the experimental group from the three dimensions of physical function,psychological function and social function were all lower than those in the control group(P<0.05)at the three time points 1 week after admission,on discharge and 1 week after discharge.6.There was no significant difference in SAS and SDS scores of the primary caregivers between the two groups(P>0.05).1 week after admission,on discharge and 1 week after discharge,the SAS and SDS scores of the caregivers in the experimental group were all lower than those at admission(P<0.05).The SAS and SDS scores of caregivers in the experimental group at the above three time nodes were lower than those in the control group,with statistical differences(P<0.05).7.The overall satisfaction rate of primary caregivers in the experimental group was 94.1%,higher than 75.8%(P<0.05)in the control group.The biggest difference in the satisfaction degree of primary caregivers in the two groups was reflected in the"satisfied" degree,which was 70%in the experimental group and the difference was statistically significant(P<0.09).ConclusionFamily empowerment nursing can promote the improvement of symptoms in children undergoing bronchoscopy for lobar pneumonia,reduce the incidence of adverse reactions after bronchoscopy,and can significantly improve the treatment compliance of children.The primary caregivers of children had high recognition of family empowerment.And family empowerment nursing could reduce the burden of the primary caregivers,at the same time,significantly reduce psychological anxiety,suppression and other adverse emotions of the primary caregivers,and improve the satisfaction of nursing work.
