With the improvement of living standard and quality of life,more and more people are concerned that death,as an important part of life development,should be given positive attention.As one of the public problems that seriously threaten people’s health,cancer affects people’s life quality and life health.The incidence of cancer is increasing year by year,and cancer patients are told that they are going to die without psychological preparation.Influenced by the traditional view,the lack of death education,make cancer patients for sudden death hold fear and escape attitude.Psychological and cognitive pressure not only influence the life quality of patients,but also shortens the time of survival in patients with patients in the pain and fear the last moment of life.Using the experience of medical social work internship in Z hospital in HuBei province,the author studied the development process of patients’ death cognition by participating in the observation and follow-up interviews of 12 patients with advanced cancer and using the cognitive development theory and narrative treatment theory.According to the conclusion of the study,patients’ death cognition can be roughly divided into four stages:"Denial-Anger";"Anger-Negotiation";"Negotiation-Despair";"Despair-Acceptance".When patients first learn of cancer,they tend to associate cancer with death and have feelings of "doubt" and "angry".With the further development of the disease,the patient,under the influence of the consciousness of survival,"negotiated"with the doctor and family,and "expected" to extend the life span as much as possible.After a period of "hope-disappointment",patients will feel helpless to the reality and have a "depressed" mood.As the disease progresses to an advanced stage,the patient gradually "accepts" death actively or passively after a long period of psychological adjustment.Based on the above four stages of cancer patients’ death cognition,the author conducted group work program intervention for cancer patients.The group work activity is designed as four stages:first understanding with death,digging up the positive energy of death,rebuilding the meaning of life and accepting death.Through knowledge popularization,experience reflection,discussion and sharing,we can help patients to realize the source of fear and anxiety,establish scientific cognition of death,and improve the quality of life.Throughout the whole process of research intervention,the author proposes Suggestions and reflections on the intervention of social work in the cognition of death of terminal cancer patients.At the level of self,patients are guided to transform their death cognition,and their sense of self-worth is improved and their self-confidence is rebuilt through "narrative therapy" and other methods.At the level of others,improve the attitude of relatives and patients towards death,promote communication and interaction,and relieve emotional stress.At the environmental level,we will work with all sectors of society to create a cultural atmosphere of "superior death",popularize death education,and help patients establish a scientific outlook on life.
