1.Background: In recent years,due to the change in population structure in China and the world,the prevalence of infectious and chronic diseases has increased.Dementia is a serious chronic disease,which has aroused great interest in medical and mental health research.The root causes of dementia include neurodegenerative changes,which decreased brain connections that lead to impaired decision-making and memory,and non-neurodegenerative mechanisms such as vessel disease(VD).VD is the second common dementia.With the rapid development of imaging technology,people are begining to find the specific characteristic manifestations of Cerebral small vessel disease(CSVD)in imaging,which leads to the cognitive dysfunction of CSVD.2.Objectives:The objectives of this research are the exploration of the related risk factors of CSVD regarding the cognitive impairment;the different imaging manifestations of CSVD;the correlation between the total burden of CSVD MRI and cognitive impairment,and the effects of aerobic exercise(finger exercise)on CSVD related cognitive impairment.3 Methods:From September 2017 to October 2019,CSVD related cognitive impairment patients were collected from the outpatient and inpatient department of Huaihe Hospital of Henan University.Collected the general informations of patients,the blood test and imaging examination results,and gave some patients aerobic exercise(finger exercise)intervention based on other interventional homogenization,and followed up as outpatient to study the change of their cognitive function.4.Results:The results of this research came as listed below:1.Age,LDL,HCY,hypertension and diabetes were negatively correlated with cognitive dysfunction(r =-0.171,-2.230,-0.206,-2.405,-4.468,P < 0.05).The average years of education was positively correlated with cognitive dysfunction(r =0.190,P=0.024),Logistic regression analysis showed that hypertension,diabetes and LDL were independent risk factors of CSVD(OR = 2.387,14.692,1.885,P < 0.05),HCY was not independent risk factor of CSVD(or = 0.941,P = 0.107).2.There were significant differences in WMHs,brain atrophy and the total burden of CSVD MRI between the regular group and the experimental group(P < 0.05);logistic regression analysis showed that WMHs,LI,EPVS and the total burden of CSVD MRI was an independentrisk factor for cognitive impairment(or = 8.276,8.003,3.2247,2.034),Brain atrophy was not an independent risk factor for CSVD related cognitive dysfunction(or = 1.669;P = 0.308).3.MOCA was more effective than MMSE in reflecting the real cognitive situation of CSVD patients.By analyzing scores of each sub-item of the MOCA scale,the conclusion is that CSVD causes most severe damages to cognitive domains such as visual space / executive ability,language ability,and delayed recall ability.4.WMHs,brain atrophy,EPVs,Li and cognitive impairment were correlated(r =-0.222,0.2,0.253,0.246,P< 0.05),WMHs was negatively correlated,while CMBS under the curtain was negatively correlated with cognitive score(r =-0.219,P = 0.030).5.There was a negative correlation between the total burden of CSVD MRI and cognitive score(r =-0.339,P = 0.001),and there was a difference between the 3-4 groups and 0-2 groups.6.At the same time as other homogenized interventions,76 patients were given finger exercise instruction and were followed up at 3 and 6 months to obtain a cognitive scale respectively.The results showed that there was a statistically significant difference between before and after the intervention group(F = 3.240,P = 0.041).Further analysis of the SNK-q test showed that there was no difference between baseline and three months' test,but there was a statistically significant difference after six months.5.Conclusions:1.High education years and aerobic exercise(finger exercise)are the protective factors of cognitive impairment.Aerobic exercise can delay and improve cognitive dysfunction.2.CMBS,WMHs,EPVs,and LI should be emphasized among all specific CSVD imagine method.The relationship between CSVD and cognitive impairment is more objective.
