| ObjectDescribe the duration of incontinence-associated dermatitis in the ICU;Explore the factors that affect the duration of incontinence-associated dermatitis in the ICU.MethodConvenience sampling was used to select patients with incontinence-associated dermatitis during the Department of Intensive Medicine of a hospital in Zhengzhou from July 2018 to July 2019 were selected as the research objects.The patient’s general information and clinical data such as infection,hypoxia,antibiotic were collected,and the nursing management measures as influencing factors for duration of incontinence-associated dermatitis are collected and recorded until the incontinence-associated dermatitis is healed or transferred out of the ICU.The influencing factors of the duration of incontinence-associated dermatitis were discussed by Kaplan-Meimer univariate analysis and Cox multivariate analysis.Results1.A total of 139 IAD patients were collected in this study.The incidence of patients with incontinence-associated dermatitis in the ICU was 14.1%.The study subject’s age with an average age(64.18 ± 14.10).The type of incontinence is mainly fecal incontinence.The median duration of incontinence-associated dermatitis was 2.0 days,the longest was 5.5 days,and the shortest was 1.0 days.2.Univariate analysis showed significant differences in incontinence frequency(P=0.015),infection time(P=0.001),antibiotic use time(P<0.001),mean APACHEⅡ score(P<0.001),mean Braden score(P<0.001),mean Glasgow coma scale score(P=0.002),defecation management method(P=0.016),whether to use a drainage device(P=0.032),whether to increase the rate of video surveillance(P<0.001)and so on.3.Cox regression analysis showed that incontinence frequency,Braden score,degree of conscious disturbance,severity of illness,infection time,defecation management method,whether to increase the rate of patrolling video,whether to use drainage device(x~2=8.427,P=0.015;x~2=74.06,P<0.001;x~2=26.01,P=0.002;x~2=65.26,P<0.001;x~2=32.11,P=0.001;x~2=22.18,P=0.016;x~2=17.58,P<0.001;x~2=21.74,P=0.032)is the influencing factor of the duration of factors that affect the duration of incontinence-associated dermatitis in ICU patients.Conclusions1.The incidence of incontinence-associated dermatitis in ICU patients was 14.1%,the average duration is from the shortest 1.0 days to the longest 5.5 days,and the median duration is 2.0 days.2.The duration of incontinence-associated dermatitis in patients with ICU is related to the frequency of incontinence,Braden score,defecation management method,whether to increase the rate of video surveillance,the use of drainage devices,the degree of conscious disturbance,the severity of the disease,and the time of infection.3.Medical staff should attach great importance to the management of incontinence-associated dermatitis.Strengthen the management of ICU patients with frequent incontinence,low Braden scale score,high severity of illness,and severe degree of consciousness disorder.In order to possibly shorten the duration of incontinence-associated dermatitis,medical staff can choose appropriate bowel methods and increase frequency on the regular ICU patrol video rate. |
