Objective: to review the surgical treatment of 134 patients with endometrial cancer in gynecological department of the second affiliated hospital of Dalian Medical University from January 2015 to June 2018,analyze the clinical manifestations,risk factors and pathological conditions of patients of different ages,and provide individualized diagnosis and treatment basis for patients of different ages in clinical work.Methods: A total of 134 patients with endometrial cancer who underwent surgical treatment in gynecological department of the second affiliated hospital of Dalian Medical University from January 2015 to June 2018 were collected.The clinical symptoms,related risk factors and postoperative pathological data of patients of different ages were statistically analyzed and grouped for statistical treatment.The data extracted in the study were analyzed by spss21.0 software.The counting data were expressed by frequency or percentage(%),and the parallel test or Fisher’s accurate test were conducted.The measurement data were expressed by mean ± standard deviation(±S),which was expressed by t test,p <0.05 indicated that the difference was statistically significant.Results: 1.The age of EC onset was 40-60 years old,mainly 51-55 years old,and there were more postmenopausal women.2.There were differences between the younger group and the 40-year-old premenopausal group and the postmenopausal group in terms of nulliparity,with a slightly higher nulliparity rate in the younger group.3.diabetes,hypertension,hyperlipidemia,and body mass index(BMI)>24 kg/m2 four EC risk factors,the combination of diabetes mellitus was different in the young group,the 40-year-old premenopausal group and the postmenopausal group,but there was no difference after pairwise comparison.4.Symptoms of vaginal bleeding were no different in the younger group,the 40-year-premenopausal group,and the postmenopausal group.5.There was no difference between the young group,the 40-year-old premenopausal group and the postmenopausal group in terms of ultrasound examination and pathological examination after segmentebral curettage and postoperative curettage.The preoperative carbohydrate antigen 125(Ca125)positive rate was no difference between the young group,the 40-year-old premenopausal group and the postmenopausal group.6.Type I cancer was the pathological type with high incidence,while type II cancer was different in the premenopausal and postmenopausal groups.Postoperative pathological stage 1 was the most common,and there was no difference between the young group and the 40-year-old premenopausal group and the postmenopausal group.Postoperative pathological grade and high differentiation were different in the young group and the postmenopausal group.7.Endometrial Carcinomar with adenomyosis was not different in the younger group,the 40-year-old premenopausal group and the postmenopausal group.Conclusion: 1.The proportion of young patients is slightly less,and the proportion of patients with the disease increases gradually with the increase of age,while the prevalence decreases after the age of 60.Endometrial Carcinomar is most common in postmenopausal women.2.The childless rate of young patients is slightly higher.Younger patients have a slightly higher rate of childlessness3.The proportion of risk factors for complications(including diabetes,hypertension,hyperlipidemia)and obesity in patients of different ages is slightly different.The risk factors for young patients are mainly overweight and obesity,and postmenopausal patients have all kinds of risk factors.BMI≥24kg/m2 increases the risk of endometrial carcinomar.Moreover,overweight can be improved by self-adjustment,so as to reduce the risk of endometrial carcinomar in prevention.4.Vaginal bleeding is still the main clinical symptom in endometrial carcinomar patients of different ages,and other symptoms(such as abdominal pain and vaginal discharge,etc.)are mostly found in postmenopausal women.5.Ultrasonic examination combined with diagnostic segmentational curettage combined with pathological examination plays an important role in the diagnosis of endometrial carcinomar.6.Among patients with endometrial carcinomar of different ages,type I cancer was the pathological type with high incidence after surgery,and type II cancer had a higher proportion in postmenopausal patients.Postoperative pathological stage 1was the most common,and young patients with high differentiation rate were the most common.7.The proportion of endometrial carcinomar patients with adenomyosis was 23.88%.Adenomyosis,as an estrogen-dependent disease,may be a risk factor for endometrial carcinomar,but a large number of studies are needed to prove this. |
