Objective:Diagnosis and treatment of endometrial carcinoma were summarized to explore the clinical features of endometrial carcinoma in different age stratification and pathological types and the risk factors for the metastasis of the ovary and lymph nodes,so as to provide evidence for guiding clinical work.Methods:Retrospective analysis was used to analyze the clinical data of 441 patients with endometrial cancer in general hospital of Ningxia Medical University and cardio cerebrovascular disease hospital from 2011 to 2015.Groups were grouped according to age,pathologica type,whether or not the ovarian metastasis/lymph node metastasis,and analysis of the characteristics of various clinical data in different groups.SPSS17.0 statistical software was used for statistical analysis.T test was used for measurement data.The count data are expressed in frequency or rate,used?~2 test or accurate probability method.Risk factors for ovarian metastasis and lymph node metastasis was analysed by Chi-square test.Inspection level of alpha=0.05.Result:1.Compared the data of young and middle-aged patients,the percentage of young patients with hypertension and diabetes was lower than that in elderly patients,the percentage of young patients with infertility was higher than that in elderly patients,the proportion of young patients with vaginal bleeding were higher in elderly patients,there were statistically significant differences between the two groups.The pathological data of the two groups were compared,the difference of histological grading and myometrium infiltration were statistically significant.2.Compared the data of I type and II type,The average age of patients with type I endometrial carcinoma was small.The proportion of type I with hypertension and obesity was higher than that of type II.The proportion of vaginal discharge and other symptoms in type II was higher than that of type I,and the difference were statistically significant.Compared with the two groups of pathological data,the difference of histological grading and myometrium infiltration were statistically significant.3.Univariate analysis revealed that histological grading,myometrial invasion depth,pathological type,cervical involvement,lymph node metastasis and CA125 level were associated with ovarian metastasis in endometrial carcinoma.Factors predictive of ovarian metastasis on logistic regression were lymph node metastasis and the serum of CA125?35U/mL.4.Univariate analysis revealed that histological grading,myometrial invasion depth,pathological type,cervical involvement,ovarian metastasis and CA125 level were associated with lymph node metastasis in endometrial carcinoma.Factors predictive of lymph node metastasis on logistic regression were low histological grade,deep myometrial invasion and the serum of CA125?35U/mL.Conclusion:1.Compared with the elderly,irregular vaginal bleeding was the main symptom in young endometrial cancer patients.Patients with young endometrial cancer usually have infertility,and less hypertension and diabetes.The incidence of low histology and deep myometrium infiltration were less.2.Compared with the type I endometrial carcinoma,the age of type II endometrial carcinoma was larger than that of type I,and type II endometrial carcinoma more likely to have abnormal vaginal discharge and other discomfort symptoms.Type II endometrial carcinoma was less associated with hypertension and obesity,with a high proportion of advanced patients,low histological grade and high incidence of deep myometrium infiltration.3.Lymph node metastasis and the serum of CA125?35U/mL were the predictive factors for ovarian metastasis of endometrial carcinoma.4.Low histological grade,deep myometrial invasion and the serum of CA125?35U/mL were the predictive factors for lymph node metastasis of endometrial carcinoma.
