Purpose:this topic to molecules of disease of traditional Chinese medicine and modern medicine theory as the foundation,to detect lung distension Yang deficiency water pan syndrome and phlegm heat indicates pulmonary syndrome between two syndrome groups Growth differentiation factor in serum 15(Growth differentiation factor 15,GDF-15)level as the main outcome measure,combining with TCM symptom score and blood examination,blood gas analysis,heart colour to exceed,such as auxiliary materials,explore serum GDF-15 in the two lung syndrome group and the change rule of meaning,for lung distension of dialectical classification and provides some references for the clinical treatment.Methods:according to the inclusion criteria,a total of 60 cases were included in this study,including 30 cases in the yang-deficiency syndrome group and 30 cases in the phlegm-heat syndrome group.Data of the four diagnoses of the patients on the day of admission and NYHA cardiac function grading were collected for scoring(see the attached table),and related auxiliary examinations were performed,including Brain natriuretic peptide(BNP)in blood test.Potential of hydrogen(PH),Oxygen partial pressure(Pa O2)and partial pressure of carbon dioxide(co2)in blood gas analysis Pa CO2),Pulmonary artery pressure(PAP),Left ventricular ejection fraction(LVEF),right ventricular outflow tract diameter(Internal diameter of right ventricular outflow tract,RVOT)and right ventricular diameter(right ventricular Internal dimension,RV),etc.Serum gdf-15 levels were determined by ELISA.Statistical software was used to analyze the difference and correlation between gdf-15 level and symptom score,NYHA classification of cardiac function and auxiliary examination data in the two types of lung distension.Results:1.The serum gdf-15 level of the two syndrome types was compared: the yang-deficiency water-flood syndrome group was higher than the phlegm-heat obstructive lung syndrome group,and the difference was statistically significant(P<0.01).2.Comparison of TCM syndrome score and NYHA heart function classification between the two syndromes groups: the cough symptom score of the yang-deficiency water-flood syndrome group was lower than that of the phlegm-heat obstructing lung syndrome group;The scores of cyanosis,edema and NYHA cardiac function were higher than those of the phlegm-heat obstructing lung syndrome group,with statistically significant differences(P<0.01).The score of asthma symptoms in the yang-deficiency water-flood syndrome group was higher than that in the phlegm-heat obstructing lung syndrome group,and the difference was statistically significant(P<0.05).3.The two syndrome groups were compared in terms of blood gas analysis indexes: the PH level of the yang-deficiency water-flood syndrome group was lower than that of the phlegm-heat obstructing lung syndrome group,and the difference was statistically significant(P<0.05).4.Comparison of cardiac ultrasound indexes between the two syndrome groups: the RVOT level of the yang-deficiency water-flood syndrome group was higher than that of the phlegm-heat obstructing lung syndrome group,and the difference was statistically significant(P<0.01).RV level was higher than that of the phlegm-heat obstructing lung syndrome group,and the difference was statistically significant(P<0.01).5.The serum gdf-15 level was positively correlated with cyanosis,edema symptom score and NYHA cardiac function grade in the yang-deficiency and water-panting syndrome group(P<0.01).There was a positive correlation with PAP and BNP levels,and a negative correlation with LVEF levels,which was statistically significant(P<0.05).In the phlegm-heat obstructing lung syndrome group,there was no correlation between serum gdf-15 level and cyanosis,edema symptom score,NYHA cardiac function grading,PAP,BNP,and LVEF levels(P>0.05).Conclusion:1.Gdf-15 may be involved in the pathological process of yang-deficiency and water-spreading syndrome of lung distension,which has certain reference value for the assessment of critical condition and prognosis.2.There may be different degrees of cardiac insufficiency and high pulmonary circulation resistance in the syndrome of Yang deficiency and water flooding,which may be related to the consequence of Yang qi deficiency and decline in the syndrome of Yang deficiency and water flooding,dysfunction of zang-fu organs,and involvement of the lung into the heart.
