Clinical Observation On Treating Dampness-heat And Blood Stasis Syndrome Of Chronic Prostatitis By Bitugongying Decoction Based On The “Southern Yunnan Materia Medica”

Objective The purpose of this study is to observe the clinical efficacy of treating dampness-heat and blood stasis syndrome of chronic prostatitis with Bitugongying decoction based on the “Southern Yunnan Materia Medica”,and according to the clinical experience of Professor Zhang Chunhe,combined with traditional Chinese medicine and pharmacology to analyze the drug of Bitugongying decoction,so as to provide theoretical and clinical basis for the treatment of chronic prostatitis with dampness-heat and blood stasis syndrome.In order to give full play to the characteristics of “Southern Yunnan Materia Medica”.It is promising that the drugs contained in the work and Mr.Lanmao's academic thoughts are further applied to andrology clinic.Methods A total of 66 patients with dampness-heat and blood stasis syndrome of chronic prostatitis from January 2019 to January 2020 were randomly divided into experimental group(n=33)and control group(n=66).The experimental group was treated with Bitugongying decoction,twice a day,200 ml each time,and taken warm half an hour after meals.The control group was treated with Qianliejiedu capsule(approval number:Z20020140.Manufacturer: Shijiazhuang Kedi Pharmaceutical Company.Specification:each tablet contains 0.4g,12 tablets/version,2 editions/box),2 times a day,4 tablets each time,and take it with warm water after meals.At the same time,through popular science articles and doctor's advice,we guide patients to eat a balanced diet,to avoid alcohol,to avoid greasy and spicy diet,to regular their timetable,to do not overwork or too comfortable,to avoid sedentary,standing and riding for a long time,to do not holding urine,to regular semen excretion,to keep proper exercise and adjust emotions and so on.8weeks of treatment is a course.National Institutes of Health chronic Prostatitis symptom Index(NIH-CPSI),TCM syndrome score,UPOINTS phenotypic classification score system and white blood cell count in expressed prostatic secretion were used to observe the clinical efficacy of Bitugongying decoction.Results A total of 66 patients are enrolled in the group and 64 cases are completed.There are 1 case in the experimental group and 1 case in the control group due to loss of follow-up.The age and course of disease between the experimental group and the control group are compared by T test of independent samples(P>0.05).There is no significant difference between the two groups and the results are comparable.1.After treatment,the scores and total scores of National Institutes of Health chronic Prostatitis symptom Index(NIH-CPSI),TCM syndrome score and UPOINTS phenotypic classification score system in the experimental group are decreased in varying degrees,compared with those before treatment(P<0.05),and there are significant differences between the two groups(P<0.05).2.Comparison of NIH-CPSI improvement: the experimental group is clinically cured in 5 cases(15.63%),markedly effective in 8 cases(25%),effective in 17 cases(53.13%),ineffective in 2 cases(6.25%),the total effective rate was 93.75%,the proportion of cured and markedly effective was 40.63%.In the control group,0 cases were clinically cured(0%),1 case was markedly effective(3.13%),29 cases were effective(90.63%),2 cases were ineffective(6.25%),the total effective rate was 93.75%,the proportion of cured and markedly effective was 3.13%.3.Comparison of TCM syndrome score improvement: in the experimental group,6cases were clinically cured(18.75%),19 cases were markedly effective(59.38%),7 cases were effective(21.88%),0 cases were ineffective(0%),the total effective rate was 100%,the proportion of cured and markedly effective was 78.13%.In the control group,0 cases were clinically cured(0%),6 cases were markedly effective(18.75%),25 cases were effective(78.13%),1 case was ineffective(3.13%),the total effective rate was 96.88%,the proportion of cured and markedly effective was 18.75%.4.Comparison of the scores of UPOINTS phenotypic classification score system: after treatment,the Urinary symptoms,Psychosocial abnormality,Organ specific symptoms,Infection,Neurological and systemic symptoms,Tenderness of skeletal muscles,and Sexual function symptoms were alleviated in different degrees in two groups,compared with those before treatment(P<0.05),and there are significant differences between the two groups(P<0.05).5.After treatment,the number of people with WBC counts of-/ HP,+ / HP,++ / HP and +++ / HP in the EPS of the experimental group change from 3,5,18 and 6 to 24,8,0and 0.By rank sum test,before treatment,there is no significant difference between the test group and the control group(P>0.05).After treatment,there is significant difference between the test group and the control group(P<0.05).Conclusion 1.Bitugongying decoction can effectively alleviate the lower urinary symptoms,pain and discomfort symptoms,and improve the quality of life of patients,so that the corresponding NIH-CPSI score is reduced,and the proportion of cured and markedly effective is better than that of the control group.2.Bitugongying decoction can effectively reduce the scores of TCM syndromes of dampness-heat and blood stasis syndrome in patients with chronic prostatitis and alleviate various syndromes.The total effective rate and the proportion of cured and markedly effective is better than that of the control group.3.Bitugongying decoction can effectively reduce the score of UPOINTS phenotypic classification system in patients with chronic prostatitis,and alleviate the symptoms of 7factors in varying degrees.At the same time,it also shows that UPOINTS phenotypic classification score can reflect the therapeutic effect of chronic prostatitis to some extent and can be used as a reference for clinical evaluation and observation.4.Bitugongying decoction can reduce the white blood cell count of EPS in patients with chronic prostatitis with damp-heat stasis syndrome.
