Clinical And Experimental Study Of Kangfuxin Combined With Centella Triterpenes Cream On The Stage Of Hypertrophic Scar

Objective:To observe the clinical effect of Kangfuxin combined with Centella Triterpenes Cream on the early stage of hypertrophic scars and experimental research on hypertrophic scars in rabbit ears,to explore the therapeutic effect and mechanism of Kangfuxin solution on hypertrophic scars.Methods: Sixty patients with hypertrophic scars that met the case inclusion criteria were randomly divided into two groups,30 in each group.The treatment group was given Kangfuxin combined with Centella Triterpenes Cream(2 times/day);the control group was given Centella Triterpenes Cream(2 times/day,3 months as a course of treatment).The Vancouver scar scale(VSS)was used to calculate the symptom scores of the two groups of patients before and during the first month,two months,and three months after treatment to comprehensively evaluate the clinical efficacy.The experimental research:Based on the experiment of rabbit ear hypertrophic scar,the mechanism of Kangfuxin in treating hypertrophic scar was explored.Result:the clinical observation:1.Comparison of each single VSS score group,the two groups of treatment can improve the scores in the second and third months(P<0.05);comparison between groups,itching,pain,thickness in the second The month treatment group was better than the control group,and the softness and color were better than the control group in the third month treatment group(P <0.05).Vascular distribution was similar in the two groups(P>0.05).2.Comparison within the total VSS score group,the two groups of treatment were effective on hypertrophic scars(P<0.05);comparison between groups,the effect of treatinghypertrophic scars in the second and third months after treatment was better than that in the treatment group Control group(P<0.05).3.In the third month after treatment,the difference in clinical efficacy between the treatment group and the control group is statistically significant.The effect of the treatment group was better than the control group(P<0.05).4.No adverse events occurred in both groups.Rabbit hypertrophic scar test: 1.After 28 days of treatment,each dose group of Kangfuxin solution can reduce the hypertrophic scar index(P<0.05 compared with the model group);compared with the Kangruibao positive control group,Kangfubao high-dose group The scar hyperplasia index was significantly reduced(P<0.05).2.HE staining showed a large number of fibroblasts,rich and irregularly collagenous which was observed concentric circles and nodules.The number of fibroblasts was decreased in the drug intervention groups,and the vortex nodules and collagen nodules decreased significantly;3.Immunochemical tests showed that compared with the blank group,The SHH signaling pathway factors of Shh,Patch,Smo and Gli-1 in models were increased significantly.Compared with the model group,the expressions of Shh,Patch,Smo and Gli-1in the drug intervention group were down-regulated to varying degrees.Conclusion: 1.Kangfuxin combined with Centella Triterpenes Cream has obvious effect on the early treatment of hypertrophic scars,while Kangfuxin has a synergistic effect on the relief of itching,pain,thickness,hardness,and color of hypertrophic scars,which is worthy of clinical promotion.and use.2.Kangfuxin down-regulated SHH signaling pathway activity by inhibiting Shh,Patch,Smo,and Gli-1 pathway factors,inhibiting hyperproliferation of fibroblasts,and thus inhibiting hypertrophic scar growth in rabbit ears.
