ObjectivesTo understand the current situation of quality of life and empowerment level in elderly COPD patients,to clarify the factors and existing problems of quality of life and empowerment in elderly COPD patients,and to explore the correlation between quality of life and empowerment level sq.In order to provide the basis for the future based on the theory of empowerment,targeted development of appropriate measures to improve the quality of life of elderly COPD patients.MethodsThis study mainly includes the correlation research and the qualitative study of quantitative research.Using the convenient sampling method,the quantitative study selected 350 elderly COPD inpatients who met the exclusion criteria in a grade III A hospital in Yunnan Province from December 2018 to September 2019 as the subjects.Survey tools include patient general situation questionnaire,patient empowerment scale(CES),St.George Breathing Questionnaire(SGRQ),and the establishment of Excel spreadsheets for quantitative research data entry and collation.Using SPSS25.0 software to analyze the data,the number of examples,composition compared to the sex,age and other general data of elderly COPD patients to describe;The test used anovaanalysis,two comparisons of multiple samples in the same group using LSD method,the influence factors of quality of life and empowerment level in elderly COPD patients were analyzed with multiple linear regression,and the correlation between quality of life and empowerment level spearman in elderly COPD patients was analyzed with Spearman correlation,and P 0.05 indicated that the difference was statistically significant.The quality study used the method of target sampling,selected 14 elderly COPD patients who had been included in the quantitative study as the interview subject,and interviewed the patients by using the outline of the homemade semi-structured interview.Results Were transcribed by Colaizzi 7-step analysis.Results1.A total of 360 questionnaires were distributed in this study,353 were recovered,3 non-conforming questionnaires were excluded,and 350 cases were finally surveyed,with an effective recovery rate of 97.2%.The survey subjects were more male than female,with patients aged 71 and over accounting for the majority(85.8%),patients with junior high school and below education accounted for 55.14%,patients living with their families accounted for 86%,Han patients accounted for 79.7%,patients with a course of less than 10 years accounted for 65.4%,and all patients were covered by health insurance to pay for medical care.Qualitative interviews were included in 14 subjects,9 men and 5 women,with an average age of 72.2.The average score of SGRQ for elderly COPD patients was(64.46 ± 16.96),with a score of respiratory symptoms(60.01 ±19.18),limited activity(71.70 ± 18.14),and disease effects(62.42 ±18.94).The results of multi-factor analysis of quality of life show that sex,smoking history,family oxygen therapy,combined chronic diseases,Chinese medicine certificate type,treatment confidence are the main factors affecting the quality of life of elderly COPD patients.3.The results of this study show that the elderly COPD patients empowerment scale score(150.69± 22.88),the elderly COPD patients’ empowerment level multi-factor analysis results show that education,the number of acute exacerbation softened each year is the main factor of the elderly COPD patients’ empowerment level.4.Spearman-related analysis showed negative correlation between quality-of-life scores and empowerment level scores in elderly COPD patients(r=-0.161,P< 0.01);Analysis of Spearman-related scores in the quality of life score and the dimension score of the empowerment scale showed a negative correlation between the quality of life score and the patient’s confidence score in treatment(r=-0.259,P< 0.01),and the quality of life score was negatively correlated with the peer support dimension score(r=-0.135,P< 0.05).5.Interview Results: COPD recurrent acute attack in the quality of life of elderly COPD patients,the quality of life decreased year by year,emotional disorder.psychological burden,patients have the first diagnosis missed early diagnosis time three aspects of the problem,while the elderly COPD patients quality of life of the main issues related to the lack of disease prevention and control ability,patient support system is not perfect(peer,society).Conclusions1.The quality of life of elderly COPD patients is poor,with the most severe degree of activity restriction.2.The quality of life of elderly COPD patients is affected by many factors,in addition to the elderly COPD patients gender and other non-interventional factors,special attention should be paid to patients’ treatment confidence,other systemic diseases on the quality of life of patients.Secondly,regulate home oxygen therapy,to avoid the decline in quality of life caused by improper oxygen therapy.For different types of Chinese medicine certificate to patients personalized health education,increase the sense of disease control.3.The level of empowerment of elderly COPD patients is at the medium-upper level,among which the dimensions of disease control,peer support and social support are important aspects of their relatively inadequate level of empowerment.4.The higher the level of empowerment of elderly COPD patients,the better their quality of life.Among them,improving patient’s patient’s patient’s patient’s patient’s patient’s confidence and peer support is an important aspect of improving the patient’s quality of life.5.To improve the quality of life of elderly COPD patients based on the status quo of empowerment level should pay attention to improving disease control ability,improve the support system of society,family,peer and other,and improve the support of health care workers to patients’ participation in medical activities.
