Abstract/Summary:
ObjectiveUnder the guidance of the hypothesis of "etiological toxin of chinese medicine causing disease",the study on the one hand analyzes the correlation between inflammatory factors, blood lipids and toxin syndrome in coronary heart disease and on the other, explores the physical and chemical indexes of toxin syndrome in coronary heart disease. The aim here is to offer a reference for the clinical treatment of coronary heart disease.MethodsCases of the study come from patients who suffered from chest aching and were hospitalized in the cardiovascular department of Xiyuan Hospital,China Academy of Chinese Medical Sciences, from 1 September 2017 to 30 September 2019.The results from coronary arteriography determines that 467 patients who met the diagnostic criteria of coronary heart disease are selected as the observation group, and 87 patients who did not develop coronary heart disease are selected as the control group. The observation group falls into two groups according to The Criterion of Syndrome Differentiation and Quantification for Stable Coronary Heart Disease Caused by Etiological Toxin of Chinese Medicine:toxin syndrome group and non-toxin syndrome group.General information of patients, such as gender, age, past medical history, allergic history, and their laboratory data such as platelet count, neutrophil count, lymphocyte count, platelet distribution width,homocysteine,high-sensitivity c-reactive protein,cholesterol,triglyceride,high-density lipoprotein cholesterol,low-density Lipoprotein, apolipoprotein A1,apolipoprotein B,Lipoprotein(a)and the results of coronary artery angiography is collected according to HIS system and integrated platforms of clinical research of coronary heart disease. Then the NLR,CHOL/HDL-C、TG/HDL-C、APO-B/APO-A1 and other indexes of patients are calculated.SPSS21.0 is used to analyze and make statistics of the data collected, and the observation group and the control group are compared in terms of the difference in the levels of inflammatory factors and blood lipids. Then logistic regression analysis is employed to delve into the syndrome differentiation index related to the toxin syndrome in coronary heart disease.Results(1)The NEUT of the CHD group is noticeably higher than that of the control group, and the NLR,CHOL,HDL-C and LDL-C of the CHD group are substantially lower than those of the control group. And the difference is statistically significant(P<0.05).(2)The NEUT,Hs-CRP,LP(a)of the toxin syndrome group are significantly higher than those in the control group. The CHOL,HDL-C and APO-A1 of toxin syndrome group are obviously lower than those in control group. The HDL-C and APO-A1 of the toxin syndrome group are significantly lower than those of its opposite group. The NEUT,Hs-CRP,LP(a),CHOL/HDL-C,TG/HDL-C and APO-B/APO-A1 of the toxin syndrome group are significantly higher than those of the non-toxin syndrome group(P<0.05)·(3)71.1%of the researched subjects can be correctly classified through Logistic model of toxin syndrome in coronary heart disease. The sensitivity is 60.9%;the specificity is 81.7%;the positive predictive value is 77.5 and the negative predictive value is 66.9%.Among the variables included in the model, neutrophil, high sensitive c-reactive protein, and Lipoprotein(a)are noticeably associated with toxin syndrome in coronary heart disease.Conclusions(1)The level of HDL-C and APO-A1 in toxin syndrome in coronary heart disease is substantially lower than that in the control group and the non-toxin syndrome group.(2)Toxin syndrome in coronary heart disease is positively correlated with CHOL/HDL-C、TG/HDL-C、APO-B/APO-A1.(3)Logistic regression analysis shows that the levels of neutrophil count,Hs-CRP and LP(a)in toxin syndrome in coronary heart disease is higher than those in patients without toxin syndrome. These items can be considered as objective indicators to distinguish the toxin syndrome in coronary heart disease.
