Preliminary Analysis Of Risk Factors For Urinary Sepsis After Flexible Ureteroscope Lithotripsy

Objective: To explore the risk factors of uremic sepsis after flexible ureteroscopic lithotripsy(FURL),though analyzing the clinical data of adult patients with urinary calculi treated by FURL.Methods:We retrospectively analyzed 400 cases of ureterolithotripsy holmium laser lithotripsy in our hospital from October 2018 to October2019,and met the inclusion criteria.A retrospective analysis was performed on the clinical records of 26 cases of urogenic sepsis complicated by ureteral flexible holmium laser lithotripsy and374 cases without uurogenic sepsis.The data of age,gender,body mass index,degree of hydronephrosis,percentage of neutrophils before operation,urinary leukocytes,diameter of stones,whether DJ tube is inserted before operation,diabetes,calculus polyp wrap,stone CT value,preoperative hemoglobin,neutrophils,urea nitrogen,creatinine,platelet,bilateral operation,ureteral stenosis,urine culture and other data were analyzed statistically.The variables satisfying the parameter test of measurement data are expressed with ± s,and independent sample t-test is used;the variables failing to meet the parameter test of measurement data are expressed with m(P25 ? p75),and rank sum test(Mann Whitney U test)is used;the count data are expressed with ?2 test,and the variables with statistical significance in single factor analysis are expressed with binary logistic regression analysis to evaluate the risk factors of postoperative urogenic sepsis.The difference was statistically significant at P <0.05.Results: Among the indicators for statistical analysis,gender,stone diameter,number of stones,diabetes,preoperative urine leukocyte positive,preoperative urine culture positive,previous stone surgery history,and stone polyp wrapping(P <0.05)may be the risk factors of urogenic sepsis after FURL operation.Multivariate analysis showed that diabetes(or = 4.542,95% C.I: 1.282-16.096,P = 0.019),calculus and polyp wrapping(or = 24.756,95% C.I: 7.423-82.561,P < 0.001)may be the independent risk factor for urinary sepsis after FURL.Conclusion: 1.Gender,stone diameter,number of stones,diabetes,positive white blood cells before surgery,wrapped in polyps of stones,positive urine culture before surgery,previous history of stone surgery may be risk factors for urogenic sepsis after FURL;2.Diabetes and calculus polyps may be independent risk factors for urogenic sepsis after FURL.
