To Compare CO₂ Laser Tympanostomy And Grommet Inserction In The Treatment For Secretory Otitis Media With Effusion

Objective::To compare CO₂ laser tympanostomy and grommet inserction in the treatment for secretory OME,and to provide basis for selection of surgical treatments.Method:Forty eight patients with OME were divided into 2 groups:group A 17 cases?23ears?were treated with CO₂laser tympanostomy;group B:19 cases?25ears?were treated with grommet inserction.Endotoscopy,acoustic immittance, tympanometry and PTA were conducted.All patients conducted endotoscopy,acoustic immittance,tympanometry and PTA at 1,3,6 months after the operation and they were followed up until 1 years surgery.The efficacy,postoperative complications and recurrence rate were compared between the two groups.Result:At 6 months after operation,the postoperative of A?23 cases?and B?25 cases?were 73.91%and 92.00%respectively.The differences between A and B groups were statistically significant.Postoperative complications of middle ear infection,airway obstruction or detachment were 0.86%?1 case of tympanosclerosis,1 case of permanent tympanic membrane perforation?,28.00%?2 cases of tympanic tube blockage or deatchment,1 case of middle ear infection postoperative,4 case of permanent tympanic membrane perforation.?Conclusion: For patients with OME,grommet insertion may be higher than CO₂laser tympanostomy.But considering the complication rates of the two surgical methods,it is recommended that CO₂ laser tympanostomy be performed routinely,and grommet insertion is recommended to be conducted when the symptoms recur after CO₂ laser tympanostomy.
