OBJECTIVE:To observe the impulse oscillometry(IOS)and echocardiography of patients with obstructive sleep apnea-hypopnea syndrome.To explore the correlation between IOS and PSG,echocardiography and PSG,IOS and echocardiography.To explore IOS and echocardiography diagnostic value in patients with OSAHS.An attempt was made to find a simple,non-invasive,practical,affordable and reliable auxiliary PSG to accurately screen the high-risk population of OSAHS and provide a more comprehensive patient evaluation for the diagnosis of OSAHS.For patients who have been diagnosed with OSAHS,seek a simple and reliable efficacy assessment method to assess cardiac and respiratory function,disease progression and efficacy,and assist patients in self-awareness and health management of the disease.Methods:46 patients with OSAHS were selected through medical history inquiry,physical examination and PSG,and 11 healthy people as normal control groups matched with age and gender were selected.The basic data survey,PSG,impulse oscillometry,echocardiography were collected,and data were collected and analyzed statistically to explore the relationship between the changes of cardiac and respiratory function and the degree of illness in patients with OSAHS,and the relationship between the changes of cardiac and respiratory function in patients with OSAHS.Results:1.Comparison of IOS index between the two groups,the index of Zrs,Rc,R5,R20,R5-R20 in the patient group was higher than that in the control group.2.Comparison of IOS indicators of patients with different severity,Zrs,R5 are different between mild disease group and severe disease group,Zrs and R5 increased in the severe disease group compared with the mild disease group;R20 is different between moderate disease group and severe disease group,R20 in the severe group is more than the moderate group.Rc was different among different disease groups,the heavier the disease is,the more Rc increases.3.There is a positive correlation between sleep apnea index AHI and Rc,and the correlation coefficient r=0.65.4.Comparison of cardiac structure and functional indicators between the two groups,the left ventricular posterior wall,the left ventricular internal diameter at end-diastole,and the left atrial inner diameter were higher in the patient group than in the control group.For E/A and LVMI,control group was larger than the patient group.5.Comparison of cardiac structure and cardiac function indexes of patients with different severity,the left ventricular internal diameter at end-diastole was different between the severe disease group and the mild disease group,and between the severe disease group and the moderate disease group.There were differences in the left atrial inner diameter among all the groups.6.AHI is positively correlated with the left ventricular posterior wall and left atrial inner diameter,with correlation coefficients of 0.271 and 0.347.The correlation was not affected by hypertension.7.There was no statistical significance in the correlation analysis between the measured value minus the expected value of X5>0.2k Pa/(L*s)and the echocardiographic index.Conclusions:1.Zrs,Rc,R5,R20,R5-R20 of OSAHS patients are increased compared with healthy subjects.2.There were differences in Zrs and R5 between the mild disease group and the severe disease group.Zrs and R5 increased in the severe disease group compared with the mild disease group;R20 was different between the moderate disease group and the severe disease group.R20 was increased in the severe disease group compared with the moderate disease group;Rc varied in each group of disease severity.The heavier the disease was,the more Rc increased.3.IOS is a method for detecting pulmonary function,especially upper airway resistance,in patients with OSAHS.Its index Rc can reflect the degree of OSAHS disease.4.The left ventricular posterior wall,the left atrial inner diameter,and the left ventricular internal diameter at end-diastole of OSAHS patients increased compared with healthy subjects.LVMI and E/A decreased compared with healthy subjects.5.The left ventricular internal diameter at end-diastole changed significantly in the severe disease group,which was different from the mild group and the increased in the severe group compared with the mild group.The left atrial diameter varied in each group of disease severity.The left ventricular posterior wall and the left atrial diameter were positively correlated with the severity of the disease.In general,changes in cardiac structure and cardiac function caused by OSAHS become more pronounced as the disease progresses.6.Echocardiography is a method for detecting changes in cardiac structure and cardiac function of patients with OSAHS.The indexes of the left ventricular posterior wall and the left atrial inner diameter can reflect the severity of OSAHS.7.Changes in cardiac structure and cardiac function may affect thorax and lung elastic resistance,and the correlation needs further study.
