Objective Objective To observe whether the serum thyroid stimulating hormone(TSH)level in normal range has an influence on the incidence rate of diabetes mellitus(DM)in normal glucose population.Methods Based on the REACTION study initiated by the Chinese medical association.Subjects with blood glucose level and TSH level in the normal range in2011 were selected,the number were 3809.Measuring the height,weight,waist,hip,blood pressure,and heart rate.To calculate body mass index,waist hip ratio.Laboratory testing total cholesterol,triglycerides,high-density lipoprotein-cholesterol,low-density lipoprotein-cholesterol,fasting plasma glucose,OGTT 2-hour postprandial blood glucose,glycosylated hemoglobin,fasting insulin,TSH,free triiodothyronine,free thyroxine.Without any intervention(diet,sport and drugs),the above subjects were follow-up in 2014,the number of people progressing to diabetes were 374,According to the three percentile level of TSH,the enrolled population was divided into three group: Q1,Q2,Q3;and according to the three percentile level of FT3 and FT4,the enrolled population was divided into three groups:S1,S2,S3 and three groups: R1,R2,R3.To compare the incidence rate of diabetes among different groups.Further analysis of risk factors of diabetes.Results1.The incidence rate of diabetes in three groups was compared with the groups in TSH level.There was no significant difference between Q1 and Q2(P>0.05),between Q1 and Q3,Q2 and Q3.The incidence rate of diabetes was statistically significant(P<0.05),and there was no statistically significant difference in the incidence rate of diabetes between FT3 and FT4 in three groups.2.They were divided into three groups by TSH.The baseline data of BMI,WHR,blood pressure,TC,TG,LDL-C,HDL-C,FPG,OGTT 2h PG,Hb A1 c,FINS,FT3,FT4 were compared among groups.It was found that higher TSH group had higher WHR,LDL-C,TC,TG,FINS,HOMA-IR(P<0.05).3.With “diabetes mellitus” as the dependent variable and baseline data as the independent variable to do logistic regression,the results showed that: the risk factors of DM were TC,TG,HOMA,WHR,LDL-C and TSH(P<0.05).HDL-C was protective factors in DM(P<0.05).Conclusions Even at normal levels,the high level of TSH still leads to higher incidence rate of diabetes.At the same time,the higher TSH level also has higher blood lipid level and insulin resistance index.
