Backgroud and objectivePostoperative cognitive dysfunction?POCD?is a common complication of central nervous system in the elderly after anesthesia surgery,which often occurs in the days to weeks after surgery,and mainly manifests in cognitive function changes of different degrees.Up to now,the pathogenesis of POCD is still unclear,which has become a research hotspot.A large number of studies have shown that there is a close relationship between the central nervous inflammatory response after surgery and the occurrence of POCD,and the activation of central astrocytes and microglia is an important process of the occurrence of nerve inflammatory response.Guhan Yangshengjing has been proved to enhance the ability of learning and memory,with a protective effect on the brain.So we use 20 to 22 months old rats,with the Guhan Yangshengjing oral liquid lavage pretreatment for 30 days,and then during isoflurane anesthesia laparotomy was downward POCD animal model,to explore the Guhan Yangshengjing whether can improve the rats cognitive dysfunction,its mechanism is related to inhibit the activation of astrocytes and microglia in the hippocampal CA1 region.MethodsFourty-eight sprague-dawley SD male rats aged 20-22 months were randomly divided into four groups,namely control,Guhan Yangshengjing?1.6g/kg?,surgery,and Guhan Yangshengjing?1.6g/kg?+surgery,with 12 rats in each group.Continuous intragastric pretreatment of Guhan Yangshengjing for 30 days,and rats in the other groups were treated with distilled water of equal volume for 30 days,and then POCD animal model was established by exploratory laparotomy under isoflurane anesthesia.Three days after the operation,Y-maze and novel object recognition experiments were conducted to test the cognitive functions of the elderly rats,and the GFAP and IBA-1 levels in the hippocampus of the elderly rats were detected by immunofluorescence methods on the first,third and 7th day after the operation.Results1.In the Y-maze experiment,the alternation accuracy rate of the aged rats in the surgery was significantly lower than that in the control?*p<0.05?,while the alternation accuracy rate of the aged rats in the Guhan Yangshengjing?1.6g/kg?+surgery was significantly higher than that in the surgery(##p<0.01).In the novel object recognition experiment,the new object recognition index of the old rats in the surgery was significantly lower than that in the control(**p<0.01),while the new object recognition index of the old rats in the Guhan Yangshengjing?1.6g/kg?+surgery was significantly higher than that in the surgery(###p<0.001).All in all,the results showed that the elderly rats after anesthesia showed cognitive dysfunction,and the Guhan Yangshengjing had the effect of improving POCD in the elderly rats.2.The expression levels of GFAP and IBA-1 in the CA1 region of hippocampus were detected by immunofluorescence.The results showed1 and 3 days after surgery,the GFAP expression level of the aged rats in the surgery was significantly higher than that in the control,and the difference was statistically significant?*p<0.05?.The GFAP expression level of the old rats in the Guhan Yangshengjing+surgery was significantly lower than that in the surgery,and the difference was statistically significant?#p<0.05?.There was no significant difference in GFAP expression between the groups 7 days after surgery.1 and 3 days after surgery,the expression level of IBA-1 in the surgery was significantly higher than that in the control,and the difference was statistically significant(**p<0.01,***p<0.001).The expression level of IBA-1 in the old rats in the Guhan Yangshengjing+surgery was significantly lower than that in the surgery,and the difference was statistically significant(##p<0.01,####p<0.0001).There was no significant difference in IBA-1 expression between the groups 7 days after surgery.In summary,the results showed that the activation of astrocytes and microglia in the CA1 area of the hippocampus of the aged rats was enhanced after surgery,and the activation of astrocytes and microglia could be inhibited by the Guhan Yangshengjing.ConclusionGuhan Yangshengjing can improves the postoperative cognitive dysfunction of the aged rats,and its mechanism may be related to the inhibition the activation of astrocytes and microglia in the hippocampus.
