| Objective: To evaluate the therapeutic effect of integrated traditional Chinese and western medicine on microinflammation and malnutrition in type2 diabetic kidney disease stage III spleen and kidney qi deficiency by observing the microinflammation and malnutrition status of Yishen Bupi Decoction.Methods: In this study,60 outpatients and inpatients with type 2 diabetic kidney disease stage III spleen and kidney qi deficiency syndrome were selected.They were randomly divided into treatment group and control group,with a course of treatment of 12 weeks.The control group was given conventional basic treatment(including symptomatic and supportive treatment such as lifestyle changes,blood sugar control,blood pressure control,lipid lowering,etc.),while the treatment group was given conventional basic treatment+ Yishen Bupi Decoction Addition and subtraction.Through comparison with the control group,To observe the effect of " Yishen Bupi Decoction " on micro-inflammatory indexes(hs-CRP,IL-6,TNF-?),nutritional indexes(ALB,Pre-ALB,Hb,TSF)and changes in total scores of TCM symptoms and total therapeutic effects of TCM syndromes before and after treatment in patients with stage III diabetic kidney disease with spleen-kidney qi deficiency syndrome,so as to evaluate the effect of this therapeutic scheme on micro-inflammation and malnutrition in patients with type 2 diabetic kidney disease stage III spleen and kidney qi deficiency syndrome.SPSS 22.0 software was used for relevant statistical analysis.Results:(1)Comparison of microinflammatory indexes(hs-CRP,IL-6,TNF-a)between the two groups before and after treatment:(1)After 12 weeks of treatment,the hs-CRP level in the treatment group was significantly different from that in the control group(t=3.721,P=0.000).(2)After 12 weeks of treatment with Yishen Bupi Decoction,the IL-6 level in the treatment group was significantly different from that in the control group(t=2.333,P=0.023).(3)After treatment with Yishen Bupi Decoction,the TNF-? level in the treatment group was statistically significant compared with that in the control group(t=3.617,P=0.001).(2)Comparison of nutritional indexes(ALB,Pre-ALB,Hb,TSF)before and after treatment between the two groups:(1)After treatment with Yishen Bupi Decoction,the ALB level in the treatment group was significantly different from that in the control group(t=2.701,P=0.009).(2)After 12 weeks of treatment with Yishen Bupi Decoction,the level of Pre-ALB in the treatment group was significantly different from that in the control group(t=2.855,P=0.006).(3)After 12 weeks of treatment,the Hb level in the treatment group was significantly different from that in the control group(t=2.590,P=0.012).(4)After 12 weeks of treatment,the TSF level in the treatment group was significantly higher than that in the control group(t=2.035,P=0.046).(3)Comparison of TCM syndrome points: After 12 weeks of treatment,the comparison of TCM syndrome points between the treatment group and the control group has statistical significance(t=5.875,P=0.000).(4)Comparison of TCM syndrome curative effect: In the comparison of TCM syndrome curative effect between the two groups,the total effective rate in the treatment group is 80.0%;The total effective rate of the control group was 66.7%,and the difference was statistically significant(P=0.026).Conclusion:(1)Yishen Bupi Decoction combined with basic treatment can effectively reduce the levels of hs-CRP,IL-6 and TNF-? in patients with type 2 diabetic kidney disease stage III spleen and kidney qi deficiency,and improve microinflammatory state.(2)Yishen Bupi Decoction combined with basic treatment can effectively improve the levels of ALB,Pre-ALB,Hb,TSF in patients with type 2 diabetic kidney disease stage III spleen and kidney qi deficiency,and improve nutritional status.(3)Yishen Bupi Decoction combined with basic treatment can improve the clinical symptoms of type 2 diabetic kidney disease patients with stage III spleen and kidney qi deficiency. |
