Objective:To compare the efficacy of laparoscopic and traditional open appendectomy in the treatment of acute appendicitis. Methods:A retrospective analysis of clinical data about the patients who underwent laparoscopic (LA) (n=334) and traditional open appendectomy (n=92) (OA), including The biggest incision size, operation period coexist abdominal and pelvic lesions detection rates, drain in dwelling rate, postoperative discharge time, analgesics usage, postoperative incidence of complications, antibiotic use, hospitalization time and all costs etc. Results:Laparoscopic appendectomy for acute appendicitis needs a higher cost and has a better cosmetic effect of incision, Laparoscopic appendectomy has obvious advantages especially for acute simple appendicitis, but it is not very well for serious or complex acute appendicitis. So whether laparoscopic appendectomy has obvious advantages to open appendectomy still need further study. Conclusion:Laparoscopic appendectomy may not the best choice for acute appendicitis, we should take more aspects into consideration such as the state of the patient, economic conditions and patient’s willingness.
