Objective: Through the comparative analysis of laparoscopic appendectomy(laparoscopic appendectomy, LA) and open appendectomy (open appendectomy, OA) theclinical curative effect of laparoscopic appendectomy, compared with openappendectomy advantages and disadvantages and the clinical value.Methods: The clinical data of75cases with acute appendicitis from Aug2009toDec2012in our hospital were analyzed.Of75,45received open appendectomy(OAgroup)and30cases received laparoscopic appendectomy(LA group).The operationtime,blood loss,incision length,gastrointestinal function recovery time,postoperativeanalgesics, and abdominal and pelvic abscess formation and other complications werecollected and analyzed.Results: Operation time, laparoscopic group was43±10min, open group was40±9min, there were no statistical significance between the two groups (P>0.05);compared with the volume of hemorrhage, bleeding volume in operation of laparoscopicgroup was18±8ml, amount of bleeding in open group was20±6ml, there nostatistical significance between the two groups too(P>0.05), the size of operationincision laparoscopic group was2.5±0.2cm,open group,3.4±0.6cm,there wasobvious statistically significant difference between two groups (P <0.05),as the time ofpostoperative gastrointestinal function recovery, the laparoscopic group was16.2±6h,open group was34.4±5h, there was obvious statistically significant differencebetween two groups (P <0.05); the use of analgesics after surgery, laparoscopy groupwas16.7%, open group was53.3%, statistically significant differences between them (P <0.05); the incidence of postoperative complications, laparoscopy group was0%, theopen group17.8%, there were significant differences between the two groups (P <0.05).There was no significant difference in operation cost time and blood loss betweenLA and OA, while LA group had advantages in less incision length, fastergastrointestinal function recovery,less postoperative pain and fewer complicationscompared to OA group.Conclusions: Laparoscopic appendectomy (LA) as a minimally invasive surgicaltechniques emerging, compared with open appendectomy (OA), with smaller trauma,operation of less incision, lesser recovery time of gastrointestinal function, incisioninfection rate is low, the cost of treatment is relatively increased, which affects itsextensive. With the continuous progress of improving the people’s economic life andlaparoscopic techniques for minimally invasive surgery, people will be more and morehigh, the laparoscopic appendectomy is a safe, effective and mature operation mode, willbe accepted for the majority of patients and clinicians, to become the first choice fortreatment of acute appendicitis.
