Survival Analysis On Factors Affecting The Prognosis In Postoperative Patients With Oral Cancer

Objectives: The aim of this study was to evaluate survival situation in patients treated for oral cancer with surgery and to study the clinical characteristics of oral cancer.In addition,the study was conducted to dentify the prognostic factors among patients treated surgically for oral cancer.Which to provide relevant references for clinical treatment and improve the survival rate of patients with oral cancer.Methods: The patients treated surgically for oral cancer from January 2014 to December 2014 in a tertiary class hospital in Hunan province were recruited.The sociodemographic?clinical characteristics and health-related behaviors of all patients were collected.The operative time was considered as the starting date for calculation of the survival rate.The main outcome measure was survival time from the operative time to the end of the follow-up period,or to date of death(before the follow-up deadline).Survival was calculated by Kaplan-Meier method and log-rank test was performed for significance test of the predicted factors that affect the overall survival in univariate analysis.Cox proportional hazards models were used to assess potential prognostic factors for survival in multivariate analysis,and P < 0.05 was considered statistically significant.Results:(1)Sociodemographic characteristics and survive rate of oral cancer patients: a total of 531 patients were included in the study with a median age of 50 years old,including 435 males(81.92%)and 96 females(18.08%).Most of those affected were between 40 and 60 years old.273 patients(51.41%)were farmers.243 patients(45.76%)had received junior middle school education.The 1-year,3-year and 5-year survival rates were 93.03%,77.02% and 67.04%,respectively.(2)Clinicopathological characteristics: the most common oral cancer site was tongue(44.82%),270 cases(50.85%)present with advanced cancer(stage III and IV)at diagnosis.28 cases(5.27%)were admitted with precancerous lesions(oral mucosal fibropathy,oral leukoplakia).412(77.59%)patients received surgery alone.The lymph node metastasis rate was 35.40%(188/531),and it is common in patients with tongue cancer.In terms of performance status(PS score),145 patients(27.31%)had different degrees of poor performance status,186 patients(35.03%)reported pain;26 patients(4.90%)had nutritional risk and 144 patients(27.12%)had chronic diseases.(3)Health-related habits: The patients smoked,drank and chewed betel quid accounted for 68.55%(364/531),47.65%(253/531)and 24.11%(128/531),respectively.Most of them were between 40 and 60 years old.(4)The factors influencing prognosis: univariate survival analysis indicated that the overall survival of the patients was associated with the age,educational level,pathological type,lymph node metastasis,tumor staging,histological differentiation,body mass index(BMI),nutritional status,comorbidities,mouth/swallowing condition,performance status(PS),numerical rating scale(NRS),case classification,drinking and betel quid chewing(P< 0.05).Multivariable Cox regression analysis showed that differentiation,lymph node metastasis,PS and NRS scores,case classification and betel quid chewing were independent prognostic factors affecting oral cancer patients after surgery.Conclusions :(1)In a similar fashion to worldwide trends,oral cancer is mainly in the fourty to sixth decades of life.Differentiation,cervical lymph node metastasis,PS?NRS score and betel quid chewing have a significant effect on survival rate in oral cancer patients after oral cancer surgery.Consequent improvement in treatment strategies can be improved by the assessment of prognostic factors.(2)We emphasize the need for early recognition of the disease.It is necessary to raise raise oral cancer awareness,and earlier referral may result in improvement of survival.
