Professor Shi Suofang's Clinical Experience Of "The Time Of Shaoyang Diseases Tending To Be Cured" Based On Data Mining

Objective:The study aims to learn from Professor Shi Suofang's clinical experience of "the time of shaoyang diseases tending to be cured" and summarize his academic thoughts.Methods:According to the strict inclusion and exclusion standards of medical cases and opinions of Professor Shi,a total of 126 cases were included,which were treated by Professor Shi using the theory of "the time of shaoyang diseases tending to be cured".The database of medical case collection and storage was established by using Xminer Operation Tool V1.4 and SPSS 26.0 data mining system.The mining of data mining operation model was done by using association rules and cluster analysis.Then we systematically analyzed the respective and mutual laws of chief complaints,timing changes of symptoms and changing modes,clinical symptoms,tongue and pulse manifestations,and prescriptions in these medical cases.According to the data analysis results,we summarized Professor Shi Suofang's clinical experience of "the time of shaoyang diseases tending to be cured,from the aspects such as four examinations and medication characteristics.Results:As to the frequency analysis,it shows that cough ranks the first among the regularly changing symptoms,while "symptoms occurred regularly" among the timing-varying modes;the second is the "symptoms aggravated regularly".The first three symptoms occurred frequently are cough,fear of cold and dryness-heat.The frequency of dark red tongue is the highest among the manifestations of tongue texture,thin yellow coating among the manifestations of tongue coating.As to the pulse,thready,slippery and wiry pulses occurred successively with the highest frequence.In the prescription which patients took resulting in regularly changing symptoms during 03:00:01-05:00:00,the Chinese herbs occurred with more than 40%of the frequency are Radix Codonopsis,Radix Glycyrrhizae Preparata,Rhizoma Coptidis,Radix Angelicae Sinensis,Fructus Jujubae,Rhizoma Zingiberis,Herba Asari,Radix Aconiti Lateralis Preparata,Fructus Mume(processed with vinegar),Cortex Cinnamomi,Cortex Phellodendri(processed with salt),Zanthoxylum piperitum DC,Radix Scutellariae(raw),Rhizoma Zingiberis Recens,Radix Bupleuri,and Rhizoma Pinelliae Praeparatum.In the prescription which patients took resulting in regularly changing symptoms during 05:00:01-07:00:00,the Chinese herbs occurred with more than 50%of the frequency are Radix Glycyrrhizae Preparata,Radix Scutellariae(raw),Radix Bupleuri,Rhizoma Pinellinae Praeparata,Rhizoma Zingiberis Recens,Fructus Jujubae,and Radix Codonopsis.In the prescription which patients took resulting in regularly changing symptoms during 07:00:01-09:00:00,the Chinese herbs occurred with more than 50%of the frequency are Radix Glycyrrhizae Preparata,Radix Scutellariae(raw),Radix Bupleuri,Rhizoma Pinellinae Praeparata,Radix Codonopsis,Rhizoma Zingiberis Recens,and Fructus Jujubae.From the internal correlation analysis,the following results are obtained.In the medical cases studied,the combination of core symptoms are cough,fear of cold,dryness-heat,white and thick phlegm,and spontaneous sweating.In terms of tongue manifestation,"dark red tongue with thin coating" appears the most.In terms of pulse manifestation,"thready pulse,slippery pulse " and "thready pulse,wiry pulse" appear the most.In the prescription which patients took resulting in regularly changing symptoms during 03:00:01-05:00:00,the core Chinese herbs include:Radix Codonopsis,Rhizoma Coptidis,Herba Asari,Radix Angelicae Sinensis,and Radix Glycyrrhizae Preparata.In the prescription which patients took resulting in regularly changing symptoms during 05:00:01-07:00:00,the core Chinese herbs include:Radix Bupleuri,Radix Scutellariae(raw),Rhizoma Pinellinae Praeparata,Rhizoma Zingiberis Recens,and Radix Glycyrrhizae Preparata.In the prescription which patients took resulting in regularly changing symptoms during 07:00:01-09:00:00,the core Chinese herbs are:Radix Bupleuri,Radix Scutellariae(raw),Rhizoma Pinellinae Praeparata,and Radix Glycyrrhizae Preparata.According to K-means cluster analysis,we get eight Chinese medicine combinations,which are divided into two groups of core formulas:Formula 1:Fructus Mume(processed with vinegar),Herba Asari,Cortex Cinnamomi,Rhizoma Coptidis,Cortex Phellodendri(processed with salt),Radix Angelicae Sinensis,Radix Codonopsis,Zanthoxylum piperitum DC,Rhizoma Zingiberis Radix Aconiti Lateralis Preparata,Radix Bupleuri,Radix Scutellariae(raw),and Radix Glycyrrhizae Preparata.Formula 2:Radix Bupleuri,Radix Scutellariae(raw),Rhizoma Pinellinae Praeparata,Rhizoma Zingiberis Recens,Radix Codonopsis,Fructus Jujubae,and Radix Glycyrrhizae Preparata.Conclusion:In this study,among the patients whose symptoms change regularly during the period of "shaoyang diseases tending to be cured",the symptom with regular changes is mainly cough,which often appear regularly from 03:00:01-09:00,and can also be accompanied by other discomforts such as fear of cold,dryness-heat,white and thick phlegm and spontaneous sweating.The tongue is mostly dark red with thin cotaing,and the pulse is mostly slippery or thready and wiry.We call this special cough "Shaoyang cough".Professor Shi uses Minor Bupleurum Decoction as the basic prescription based on the theory of "the time of shaoyang diseases tending to be cured",and modifies it separately according to the timing changes of symptoms.For example,when patients'time-changing symptoms appear regularly from 03:00:01-05:00,Mume Pill is often added and modified;from 05:00:01-09:00,Minor Bupleurum Decoction is often selected.When symptoms occur during the time period of "other meridian diseases tending to be cured",prescriptions that are often used in other meridians are used or added.For patients with concurrent pathogenesis,the corresponding TCM prescriptions are used.
