BackgroundCoronary atherosclerotic heart disease is one of the most common type of diseases caused by atherosclerosis,and it is also a common disease that seriously endangers people's health.It occurs more frequently after middle age,and males are more susceptible.Age is an important risk factor for coronary heart disease.Framingham risk score study shows that the absolute risk of coronary heart disease increases with age.With the aging of population,the incidence rate and mortality rate of cardiovascular disease in the elderly population has been increasing year by year.It has become the first cause of death in China.How to standardize the diagnosis and treatment and improve the success rate of treatment,improve the quality of life and long-term survival rate of patients with coronary heart disease is a severe challenge facing the acceleration of aging society in China.Inflammation theory shows that atherosclerosis is a systemic,lipid driven immune inflammatory disease.Chronic low-grade inflammation plays a key role in the occurrence and development of atherosclerotic plaque,which will gradually lead to plaque instability and thrombosis,and cause adverse cardiovascular events.In addition,coronary artery disease is widely associated with neuroendocrine system and oxidative stress.At present,there are many related researches at home and abroad that have explored many new derivative indexes of Hematology test.Derivative indexes are new indexes which is obtained by calculating or adding restriction on existing indexes.The process of calculating or adding restriction is the process of refining or fusing information,which can make the index more suitable for a specific problem,and also make people easier to intuitively recognize and understand.Therefore,most of these indexes can better reflect the above pathological and physiological changes,and better predict coronary artery disease.It can also let the users of the test sheets get information which is more closely related to coronary artery disease.They include blood cell count related derived indicators,blood biochemical related derived indicators and so on.At present,most of the studies on these hematology derived indicators are relatively scattered,and there are large differences in the conditions,races and regions of various studies,which make them difficult to compare with each other.Therefore,there is a lack of systematic understanding of these indicators.In this study,we studied the relationship between the above indexes and coronary artery disease in Shandong Province,and compared their relative values,in order to provide new ideas for the prevention and treatment of coronary heart disease.MethodsAccording to the inclusion criteria and exclusion criteria,the clinical data of patients who were hospitalized in Qilu Hospital of Shandong University from January 2013 to April 2017 with suspected or confirmed coronary heart disease and underwent percutaneous coronary angiography were collected.Statistical methods such as difference analysis and logistic regression were used to analyze the relationship between various indexes and the severity or type of coronary artery lesion.Results1.The blood test indexes related to the degree of coronary artery disease included:leukocyte,plateletcrit,hemoglobin,hematocrit,monocyte,platelet distribution width,lymphocyte ratio,neutrophil,potassium,total cholesterol,triglyceride,high-density lipoprotein,low-density lipoprotein,uric acid,albumin,alkaline phosphatase,globulin,prealbumin,apo A1,apo B,fibrinogen.2.The blood lipid related derivative indexes THR,LHR and nHHR are all closely related to coronary artery lesion,and their value of reflecting the degree of coronary artery disease is higher than the original indexes in the blood test results.It stably has similar value for population of different ages,genders and diagnoses.3.The ratio of apoB/apoAl is closely related to coronary artery disease.The value of reflecting the degree of coronary artery disease in female patients is much higher than that in male patients.For non myocardial infarction patients and elderly patients,the significance was much higher than that of myocardial infarction patients and young and middle-aged patients.4.The blood cell count related derivative index NLR has obvious value for female patients and young or middle-aged patients.MLR has obvious value in female patients and in elderly patients.MHR has significant value for different ages,genders and non myocardial infarction patients,especially for women and elderly patients.5.GAR,GPAR,FAR and FPAR are valuable for patients of different ages,genders and diagnoses.The value of GAR in women and young or middle-aged patients was significantly higher,which was equivalent to the above-mentioned lipoprotein related derivative indexes.The value of GPAR is higher for myocardial infarction,male and young or middle-aged patients,especially for young or middle-aged patients,FAR is more valuable for myocardial infarction,women and young or middle-aged patients,especially for female patients,which is much higher than that of male patients,and also higher than the above-mentioned lipoprotein related derivatives-FPAR was the highest value index in the whole and most subgroups,only lower than apoB/apoAl ratio and FAR in female subgroups.6.Prealbumin and hematocrit are protective factors of coronary artery disease.Prealbumin has significant value for patients of different ages,genders and diagnoses.Hematocrit is valuable for patients of different genders,diagnoses and the elderly,and too high values for non myocardial infarction patients and the elderly will weaken the protective effect.ConclusionThe indexes in this study have a certain guiding value for the evaluation of the degree and type of coronary artery lesions.Some of the derivative indexes have certain advantages over the original indexes,and have clinical application and further research value.
