Clinical Study On Treating Vertigo Caused By Transcient Ischemia Attack Of Posterior Circulation Through The Theory Of Kidney Deficiency And Yang Hyperactivity

Objective:Analyzing Pathogenesis in syndrome differentiat as well as combining disease and syndrome,to explore the ideas and methods for the treatment of vertigo,the point is the argument basis and the syndrome treatment rule of vertigo caused by transcient ischemia attack of posterior circulation treated by the theory of kidney deficiency and yang hyperactivity,observe and evaluate the clinical effect of tonifying kidney and smoothing liver on vertigo caused by transcient ischemia attack of posterior circulation,It has important research significance for guiding the clinical treatment of and improving the clinical efficiency of this disease.Methods:By systematically reviewing and analyzing the ancient and modern literature on the understanding of vertigo caused by transcient ischemia attack of posterior circulation,combined with the tutor's experience in treating vertigo for more than 30 years,considered that vertigo caused by transcient ischemia attack of posterior circulation is characterized by kidney deficiency and yang hyperactivity as the basic pathogenesis,therefore,self-make the "prescription of tonifying kidney and smoothing liver" for the treatment of kidney deficiency and yang hyperactivity.According to the basic principles of clinical experiment design,98 patients with transcient ischemia attack of posterior circulation were divided into treatment group(49 cases)and control group(49 cases).The patients in both groups were given routine treatment in western medicine according to their individual conditions,49 patients in the treatment group is treated with prescription of tonifying kidney and smoothing liver on the basis of treatment,and the control group is treated with flunarizine hydrochloride capsule(sibelium)on the basis of treatment.The two groups are treated with 4 weeks as a course of treatment,2 consecutive courses,a total of 8 weeks.The relevant indexes are recorded and observed before treatment,after treatment and during follow-up,through comparing and analyzing the score of vertigo rating scale(DARS),vertigo disorder scale(DHI),vertebrobasilar system ischemic neurological dysfunction scale and vertigo kidney deficiency yang hyperactivity syndrome integral scale of the two groups before and after treatment,evaluate the comprehensive efficacy of prescription of tonifying kidney and smoothing liver in the treatment of vertigo caused by transcient ischemia attack of posterior circulation,so as to prove "kidney deficiency and yang hyperactivity" is the basic pathogenesis of the disease.Results:(1)The effect of vertigo caused by transcient ischemia attack of posterior circulation:in the treatment group,18 cases are cured(36.73%),23 cases are markedly effective(46.94%),7 cases are effective(14.29%),1 case is ineffective(2.04%);in the control group,1 case is cured(2.04%),28 cases are markedly effective(57.14%),17 cases are effective(34.69%),3 cases are ineffective(6.13%).The total effective rate of the treatment group is 48 cases(97.96%);the total effective rate of the control group is 46 cases(93.88%).The curative effect of vertigo in the treatment group was significantly better than that in the control group(P<0.01),and the basic recovery rate of vertigo in the treatment group was significantly better than that in the control group(P<0.01).(2)The traditional Chinese medicine syndrome effect of vertigo of kidney deficiency and yang hyperactivity type:in the treatment group,0 cases are cured,16 cases are markedly effective(32.65%),32 cases are effective(65.31%),1 case is ineffective(2.04%);the total effective rate of the treatment group is 97.96%;in the control group,0 case is cured0,0 cases are markedly effective,26 cases are effective(53.06%),23 cases are ineffective(46.94%);the total effective rate of the control group is 53.06%.The overall comparison between the two groups,the treatment group was significantly better than the control group(P<0.01).In the treatment group,the efficiency of TCM syndrome manifestation and total effective rate were significantly better than that of the control group(P<0.01).(3)Improvement of scores of various scales:After treatment,the DARS scores of the two groups were significantly lower than before treatment(P<0.01).Compared with the two groups,the scores of the treatment group were significantly lower than that of the control group(P<0.01).After treatment,the DHI scores of the two groups were significantly lower than before treatment(P<0.01).Compared with the two groups,the scores of the treatment group were significantly lower than that of the control group(P<0.01).After treatment,the scores of vertebrobasilar system ischemic neurological dysfunction scale of the two groups were significantly lower than before treatment(P<0.01).Compared with the two groups,the scores of the treatment group were significantly lower than that of the control group(P<0.01).Conclusion:The treatment effect of the prescription of tonifying kidney and smoothing liver on vertigo caused by transcient ischemia attack of posterior circulation is obviously better than flunarizine of the control group.The prescription of tonifying kidney and smoothing liver can significantly reduce the severity of vertigo and the number of vertigo attacks,as well as improve the vertebrobasilar system ischemic neurological dysfunction and the vertigo syndrome of kidney deficiency and yang hyperactivity,and that is safe without obvious adverse reaction.It isn't only proved that the kidney deficiency and yang hyperactivity treatment for vertigo caused by transcient ischemia attack of posterior circulation is effective,but it also embodies the advantages of syndrome differentiation and treatment and holistic treatment of in traditional Chinese medical.
