Research Objectives:(1)To explore the relationship between the incidence of heart failure in ICU and the concentration of PM2.5 and air quality index(AQI),and to study whether the air environment is related to the incidence of heart failure in ICU.(2)To systematically evaluate the protective effect of qiliqiangxin capsule on animal models of heart failure,and to provide evidence-based medicine basis for clinical application and experimental theory of qiliqiangxin capsule(3)experimental aspects:to explore the effect of qiliqiangxin capsule on H9C2 of rat myocardial cells under PM2.5 injury,to provide certain experimental theoretical basis for clinical application of qiliqiangxin capsule on cardiac injury.Research Methods:(1)To explore the correlation between PM2.5 and the incidence of heart failure:Collect ICU inpatients in the North Hospital of Jiangsu Provincial Hospital of Traditional Chinese Medicinefrom 2014 to 2018,and obtain monthly corresponding air quality index(AQI)and PM2.5 concentration data fromNanjing Environmental Monitoring Station from 2014 to 2018,draw scatter plots to study the relationship between PM2.5 and the incidence of heart failure patients,and group the AQI data and make statistics and analysis on the difference between air quality index(AQI)index and the incidence of heart failure among different grades.(2)To evaluate the protective effect of Qiliqiangxin capsule on heart failure animals(rat models):Through Chinese databases such as CNKI,Wanfang,Baidu Academy and VIP,the beneficial animal experiments of Qiliqiangxin capsule on myocardial injury in rats were searched.The search time was from January 1,2000 to February 1,2019.According to the inclusion and exclusion criteria,eligible documents were selected,and then relevant effective data in the documents were extracted for Meta-analysis.(Revman5.3 is the software used)(3)To explore the effect of Qiliqiangxin capsule on H9C2 cells of rat myocardial cells damaged by PM2.5:the best experimental model-making concentration of PM2.5 was screened out by CCK8 method,the appropriate concentration of Qiliqiangxin capsule was screened out by CCK8 method,and then the effect of Qiliqiangxin capsule on H9C2 cells of rat myocardial cells damaged by PM2.5 was explored by CCK8 method.Research results:(1)PM2.5 concentration is positively correlated with the incidence of heart failure in ICU(r=0.758).the higher the air quality index(AQI),the higher the incidence of heart failure.the difference between AQI of different grades and the incidence of heart failure is statistically significant(p<0.05).(2)According to Meta-analysis,the LVEDD of Qiliqiangxin capsule group was lower than that of the control model group[MD=-1.76,95%CI(-2.61--0.91)],higher than that of the sham operation group[MD=2.33,95%CI(1.19-3.46)],LVEF was higher than that of the control model group[MD=15.09,95%CI is(7.88-22.30)],lower than that of the sham operation group(MD=-13.81,95%CI is(-17.57-10.06)],BNP is lower than that of the control model group(MD=-143.14,95%Cl is(-255.09-31.18)],higher than the sham operation group(MD=24.11,95%Cl is(0.46-47.75)],Angll was lower than control group[=-69.02,95%CI(-159.87-20.48)],higher than sham operation group[MD=46.82,95%CI(34.99-58.65)],LVESD was lower than control model group[MD=-0.67,95%CI(-1.27-0.08)],higher than sham operation group[MD=0.97,95%CI(-0.65-2.58)].(3)Experimental aspects:1)After the H9C2 cells were injured for 2 hours by setting the concentration range of PM2.5 at 0.2mg/ml,0.4mg/ml,0.6mg/ml and 0.8mg/ml respectively,it was found that when the concentration of PM2.5 was 0.2mg/ml,the cell activity was the highest and the cell activity inhibition rate was close to 50%.2)respectively set the concentration range of Qiliqiangxin capsule at 0.01mg/ml,0.05mg/ml,0.1mg/ml,0.5 mg/ml,1.0 mg/ml,2.0mg/ml,and incubate H9C2 cells for 24 hours.it is found that the cell activity is the strongest when the concentration of Qiliqiangxin capsule is 0.1mg/ml,the cell activity increases when the concentration is 0.01-0.1mg/ml,and decreases when the concentration is 0.1-2.0mg/ml.3)Qiliqiangxin capsule low dose group(0.05mg/ml)can improve H9C2 cell activity under PM2.5 injury,and the difference between the groups is statistically significant(P<0.05).Conclusion:(1)PM2.5 may be a health factor leading to heart failure.(2)Qiliqiangxin capsule has protective effect on rats with heart failure.However,due to the small sample size included in the study and the large heterogeneity of the data,more experimental studies are needed in the later stage for further demonstration.(3)Through the establishment of H9C2 model of rat myocardial cells damaged by PM2.5,it is found that Qiliqiangxin capsule can protect H9C2 cells damaged by PM2.5 to some extent,but it is closely related to its concentration.
