objective To observe the clinical effect of Taiji Tuina on neck pain;to explore the changes of the analgesic effect of Taiji Tuina at different time points;to screen and identify the differential expression protein,related function and biology of Taiji Tuina in the serum of patients with neck pain by using the technology of i TRAQ,and to explore the biological basis of its therapeutic effect.Method The trial was conducted from June 2019 to December 2019,and the recruitment sites were Hubei Provincial Hospital of Traditional Chinese Medicine and Hubei University of traditional Chinese medicine.60 subjects who met the diagnostic criteria and inclusion criteria of neck pain were randomly divided into group A and group B.20 healthy people were recruited from the physical examination center as the control group.This trial is divided into two parts.One is clinical study.Group A is treated with heatwrap therapy,group B with Tuina.Each treatment lasts for 20 minutes,once a day,7 times in total,and is completed within 10 days.The time points of vas scale were recorded: to before treatment;T1 immediately after the first treatment;T3 immediately after the third treatment;T5 immediately after the fifth treatment;T7 immediately after the seventh treatment,a total of 5 observations.The time point of NDI scale was recorded before treatment and immediately after the seventh treatment,a total of 2 observations were made.The vas and NDI scores of the two groups at different measurement time points were compared among groups;the vas and NDI scores of the same group at different measurement time points were compared within groups.The second is the mechanism study.Three serum samples were collected from healthy control group,three before massage treatment and three after massage treatment in group B.after removing high abundance protein from serum,i TRAQ labeling and grouping were used,combined with liquid chromatography-mass spectrometry,go library analysis,to compare the number of serum differential proteins,analyze the biological process,cell components and protein functions involved in differential proteins.Result Clinical research results 1.Basic information A total of 100 subjects were recruited,12 of them fell off in the three groups,with a fall off rate of 12%.Among them,32 samples could be calculated in the hot compress group,33 in the Tuina group,23 in the health control group,with a total of 88 people,including 32 men and 56 women.The statistical results without statistical difference within two groups.(P > 0.05).2.VAS score results Comparison between the two groups: T0: No statistical difference between group A and group B(P > 0.05).T1: VAS score: group A<group B(P < 0.05).T3: No statistical difference between group A and group B(P > 0.05).T5: VAS score: group A>group B(P < 0.05).T7:VAS score: group A>group B(P < 0.05)Comparison between the two groups: In heatwrap therapy and Tuina group,the VAS score t0 > T1 > T3 > T5 > T7,the difference was statistically significant(P;Take the difference of value of VAS score t0-t0 as the benchmark,and make statistical analysis on the difference of D1 = t1-to,D2 = t3-t1,D3 = t5-t3,D4 = t7-t5;After statistical analysis of group A,D3 > D4 > D2 > D1,the difference of VAS score measured after the fifth heatwrap therapy was the largest,and the difference curve was relatively flat.The results of group B were D3 > D2 > D4 > D1.After the fifth Tuina treatment,the difference of VAS score was the largest,and the peak value of Tuina treatment difference curve was more obvious than heatwrap therapy.3.NDI scores results Part1: T7: No significant difference in NDI within A and B(P > 0.05).Part2: T7: the NDI score of group A was T7 < T0,indicating that the NDI score after 7 times of heatwrap therapy was lower than that of baseline measurement(P < 0.05).Group B NDI score T7 < t0 significance(P < 0.05).It shows that the NDI score of Tuina group after 7 times of treatment is lower than that before treatment.2.Mechanism research results Compared with the control group,the pain group had 79 differential proteins,39 up-regulated proteins and 40 down regulated proteins.Compared with the control group,there were 95 differential proteins in the pain therapy group,39 up-regulated and 56 down regulated Compared with the pain group,the number of differential proteins in the pain therapy group was 49,up regulating 49 differential proteins,including 15 up regulating proteins and 34 down regulating proteins.According to the function of protein,cell components and specific biological processes involved,the differential protein participated in 23 kinds of biological processes,in which the biological adhesion function was only involved in down regulated protein,and the other 22 kinds of biological processes were involved in up-regulated and down regulated protein.Eight cell components,up-regulated protein and down-regulated protein were involved.Among the eight molecular functions,the up-regulated protein did not participate in three molecular functions: molecular transduction activity,antioxidant activity,and cell membrane environment,and the rest were involved.conclusion 1.Tuina treatment and damp heat compress treatment can effectively improve the pain and cervical dysfunction of adolescent patients with neck pain.2.The cumulative effect of Tuina is better than that of hot compress.3.With the increase of Tuina times,the pain effect of Tuina treatment shows the trend of first effect-increasing-platform period-slowing down-cumulative effect.4.Before and after Tuina treatment,there are many kinds of cell components,biological processes and molecular functions of different proteins.Tuina treatment of neck pain is the result of multi-channel comprehensive effect.
