Objective:The clinical effects of warm acupuncture and moxibustion in treating knee osteoarthritis with symptoms of wind cold and wetness were evaluated by observing the changes of the Lequesne scores and knee flexion activity during and after treatment.Methods:All subjects who met the inclusion criteria but did not meet the exclusion criteria were randomly divided into warm acupuncture group,moxibustion group and TDP group(24 cases in each group).The warm acupuncture group was treated with warm acupuncture,the moxibustion group was treated with acupuncture combined with moxa box moxibustion,and the TDP group was treated with acupuncture combined with TDP irradiation.All subjects received treatment every Monday,Wednesday,and Friday for about 30 minutes each for a total of 4 weeks.Before treatment,after 2 weeks of treatment,and after 4 weeks of treatment,the Lequesne scores evaluation and knee flexion activity measurement were performed.Clinical efficacy was evaluated after 4 weeks of treatment.Results:1.According to statistical analysis,before treatment,the general information of the three groups of subjects(sex,age,height,weight,course of disease,etc.),Lequesne scores,knee flexion activity and other indicators were not statistically significant(P>0.05),are comparable.2.Comparison within the groups: Compared with before treatment,the Lequesne total scores and various scores of the three groups of subjects were significantly decreased after 2 weeks of treatment and 4 weeks of treatment,and the differences were statistically significant(P<0.01);The Lequesne total scores and various scores still significantly decreased after 4week of treatment,and the difference was statistically significant(P<0.01).The knee flexion activity of the three groups of subjects was significantly improved after 2 weeks and 4 weeks of treatment,and the difference was statistically significant(P<0.05).Compared with 2 weeks of treatment,the knee flexion activity was still significantly improved after 4 weeks of treatment.The difference was statistically significant(P<0.05).3.Comparison among groups: In the warm acupuncture group,the total scores of Lequesne for 2 weeks was lower than that in the moxibustion group and TDP group(P<0.05,P<0.01).In the warm acupuncture group,the total scores of Lequesne for 4 weeks was still lower than that of the moxibustion group and TDP group(both P<0.01));There was no significant difference in the total scores of Lequesne in the moxibustion group and the TDP group after 2 weeks of treatment and 4 weeks of treatment(all P>0.05).In terms of pain and discomfort,the Lequesne pain and discomfort scores after 2 weeks of treatment in the warm acupuncture group was significantly lower than that of the moxibustion group and the TDP group(both P<0.01).The Lequesne pain and discomfort scores after 4 weeks of treatment in the warm acupuncture group was significantly lower than that of the moxibustion group and the TDP group(both P<0.01).There was no significant difference in Lequesne pain and discomfort scores between the moxibustion group and the TDP group after 2 weeks of treatment and 4 weeks of treatment(both P>0.05).In terms of walking ability,there was no significant difference in Lequesne walking ability scores between the warm acupuncture group and moxibustion group after 2 weeks of treatment(P>0.05),but the scores were lower than those of the TDP group(P<0.01,P<0.05).There was no significant difference in Lequesne walking ability scores between the warm acupuncture group and moxibustion group after 2 weeks of treatment(P>0.05),but the scores were lower than those of the TDP group(P<0.01,P<0.05).In terms of viability,there was no significant difference in the Lequesne viability scores between the warm acupuncture group and the moxibustion group after 2 weeks of treatment(P>0.05),but the scores of the two groups were lower than those of the TDP group(P<0.01,P<0.05).After 4 weeks of treatment in the warm acupuncture group,the Lequesne living ability scores was lower than that in the moxibustion group and the TDP group(both P<0.01).There was no significant difference in treatment between the moxibustion group and the TDP group(P>0.05).There was no significant difference in knee flexion activity between the warm acupuncture group and the moxibustion group and the TDP group after 2 weeks of treatment(all P>0.05).After 4 weeks of treatment in the warm acupuncture group,the flexion activity of the knee joint was not significantly different from that in the moxibustion group and the TDP group(all P>0.05).There was no significant difference in the flexion activity of the knee joint between the moxibustion group and the TDP group for 2 weeks and 4 weeks(both P>0.05).4.After 4 weeks of treatment,5 cases in the warm acupuncture group were cured,14 cases were markedly effective,2 cases were effective,and 2 cases were ineffective.10 cases,5 cases of ineffectiveness,the more effective rate was 37.5%,the total effective rate was 83.33%;TDP group cured 1 case,5 cases were effective,11 cases were effective,7 cases were ineffective,the cure rate was 25%,the total effective rate 70.83%.Compared with the moxibustion group and the TDP group,the clinical effect of the warm acupuncture group was significantly different(P<0.05,P<0.01),and there was no significant difference between the moxibustion group and the TDP group(P>0.05).5.All subjects in this trial had stable vital signs during the treatment process,and did not experience any adverse conditions such as halo,broken needles,burns,and allergies.Conclusion:1.Warm acupuncture,acupuncture combined with moxa box moxibustion,and acupuncture combined with TDP irradiation have certain effects in treating KOA with symptoms of wind cold and wetness.Among them,warm acupuncture has the best effect.2.Compared with acupuncture combined with moxa box moxibustion and acupuncture combined with TDP irradiation,warm acupuncture can more effectively improve the clinical symptoms of patients,especially in terms of alleviating pain and discomfort and improving living ability of patients.In terms of improving walking ability,warm acupuncture and acupuncture combined with moxa box moxibustion have comparable improvement capabilities,and both are better than acupuncture combined with TDP irradiation.3.Warm acupuncture is effective in treating KOA with symptoms of wind cold and wetness,and it has good safety.
