| Objective To observe the clinical effects of Yang yuan tong luo acupuncture and moxibustio in the treatment of diabetic gastric motility disorder.Analyze the curative effect and its mechanism in order to provide the basis for clinical application.Methods 40 cases of diabetic gastric motility disorder which used to received treatment in Wuhan hospital integrated traditional Chinese and western medicine during june 2019 and december 2019 were divided randomly into treatment group and control group.The treatment group was given Yang yuan tong luo acupuncture and moxibustion once a day while the control group used domperidone on the basis of basic treatment including diabetes health education,relational diet and using hypoglycemic agents or insulin correctly,both treatment for two weeks.The treatment group acupuncture Baihui point,Guanyuan point,Qihai point,Yaoyangguan point,Mingmen point,Ganshu point,Shenshu point,Pishu point,Weishu point,Zusanli point,Zhongwan point,Xiawan point,Taichong point,Yanglingquan point,Sanyinjiao point,the needling depth being 0.5-1cun,while the Pishu point and Shenshu point are given moxibution.Use the routine acupuncture manipulation and retain the needle for 30 minutes while twist and turn every 10 minutes in mild reinforcing and attenuating.The range is 0.3-0.5cm and the frequency is 60-90 permin/min of lifting and thrusting method.Use an equal approach in insertion and twisting the needles.Observe the changes of TCM syndromes,clinical effects,gastric emptying rate,the electrogastric rhythm,gastric electrical amplitude and frequency,gastrointestinal hormone level including motilin,gastrin and somatostain in the two groups before and after treatment.Examine blood routine examination,urinalysis,stool routine,hepatic and renal function,electrocardiogram of patients before and after treatment,recording adverse reaction during the treatment.Finally,analysis the result by spss22.0,and evaluate the clinical efficacy of each group.Results 1.Comparison of general condition :There are no statistical significance in age,sex composition ratio,course of disease symptoms integral,gastric emptying ratr,electrogastrogram,gastrointestinal hormone level of two groups(P>0.05).2.Comparisons of efficacy index:(1)Comparison of TCM syndromes:The curative effect of symptoms including epigastric distention,abdominal fullness,abdominal noisy,inappetence,belching and acid swallowing,nausea and vomiting all relieved than before treatment in two groups(P <0.05).The score of TCM syndromes including epigastric distention,abdominal fullness,abdominal noisy,inappetence,belching and acid swallowing,nausea and vomiting in the treatment group was lower than the control group.(2)Comparison of clinical efficacy:In the treatment group,9cases were cured,6 cases markedly effective,4 cases effective,1 case ineffective,whih total effective rate was95.00%.In the control group,3 cases were cured,5 cases markedly effective,6 cases effective,6 case ineffective,whih total effective rate was70.00%.The higher the efficiency of the treatment group,the result was significant(P<0.05).(3)Comparison of gastric emptying rate:Gastric emptying rate was higher than before treatment in both group(P <0.05).The treatment group was significantly higher than the control group.(4)Changes of electrogastrogram:The electrogastric rhythm,gastric electrical amplitude and frequency were higher than before treatment in two groups(P <0.05).The treatment group was significantly higher than the control group.(5)Comparison of gastrointestinal hormones:The level of motilin and gastrin improved than before treatment in both group(P <0.05).The treatment group was significantly superior to the control group(P <0.05).The level of somatostain was lower than before treatment in both groups(P<0.05).The treatment group was significantly lower than the control group(P <0.05).3.Safety comparison:There were no obvious abnormality in blood routine examination,urinalysis,stool routine,hepatic and renal function,electrocardiogram of all patients before and after treatment.Conclusions Yang yuan tong luo acupuncture can relieve the TCM syndromes,improve clinical efficiency,enhance gastric emptying,increase the electrogastric rhythm,gastric electrical amplitude and frequency,increase the concentration of motilin and gastrin,decrease the concentration of somatostain.The effect is exact of Yangy yuan tong luo acupuncture moxibustion in the treatment of diabetic gastric motility disorder and the mechanism is possible related with the regulation of gastric electrical rhythm and the level of gastrointestinal hormones.Inaddition,this therapeutic method is safe and effient while side effect few.It is worthy of clinical application. |
