| Objective:Complicated psychological disorder after PCI of coronary heart disease belongs to the category of double heart disease.In recent years,it has gradually been paid more attention with the rise of the dual heart medical model.Some studies have found that after interventional treatment of coronary heart disease,although the blood vessels of the criminal are opened,the latter part of the patients still had clinical symptoms of chest tightness,there will even be irritability,panic,insomnia,more dreams and other mental symptoms.Yangxin Decoction comes from"Standards for Diagnosis and Treatment",the effect of clinical treatment of coronary heart disease has been significantly improved through the change of drug addition and subtraction after many years of clinical practice by the instructor.This study observes the clinical effect of Yangxin Decoction in the treatment of coronary heart disease after PCI combined with psychological disorders(qi and blood stagnation syndrome);To explore the effects of benefiting qi and nourishing blood,dispersing stagnated liver qi for relieving qi stagnation,and nourishing the heart to calm the mind in the treatment of coronary heart disease after PCI combined with psychological disorders;To explore the effects of Yangxin Decoction on TCM syndromes,mental symptoms and quality of life of patients in coronary heart disease after PCI combined with psychological disorders.Method:This study uses a clinical randomized controlled study.The cases were admitted to the Cardiology Department of Changchun University of Traditional Chinese Medicine in the inpatient or outpatient clinics from December 2018 to December 2019.The age selection was between 40 to75 years old,there were no gender requirements for men and women,72 cases through clinical diagnosis meets the diagnostic criteria of coronary heart disease after PCI combined with psychological disorders(qi and blood stagnation syndrome)were selected.Random number coding was used for random grouping,the patients were not use double blindmethod due to the small sample size in this study,the patients were divided into two groups,the experimental group and the control group,36 patients in each group.The the experimental group were given 150mL of Yangxin Decoction on the basis of conventional treatment of coronary heart disease after PCI twice aday;the control group were given flupentixol and melitracen(Deanxit)on the basis of conventional treatment of coronary heart disease after PCI once in the morning for 30 days,observe the changes in TCM syndromes,anxiety and depression scores,and quality of life scores before and after treatment in the two groups of patients.Use X~2 test,t test and other statistical methods for statistical analysis.Result:1.The total effective rate of the TCM symptom efficacy the experimental group was91.6%;the total effective rate of the control group was 80.5%,which shows that the improvement of TCM symptoms in the the experimental group was significantly better than the control group(P<0.05).2.Comparison of Hamilton anxiety and depression scores before and after treatment.The comparison between the the experimental group Yangxin Decoction and the control group Deanxit has an effect on improving anxiety and depression(P<0.05).The comparison between the the experimental group is significantly better than the control group(P<0.05),it shows that the clinical effect of the Chinese medicine Yangxin Decoction is better than the control group Deanxit in the treatment of coronary heart disease combined with psychological disorders after PCI.3.The quality of life scores of the two groups after treatment were significantly better than those before treatment,and the scores of the SF-36 self-assessment scale for the the experimental group were significantly better than control group(P<0.05).Conclusion:1.The the experimental group Yangxin Decoction and the control group Deanxit both have clinical effective in treatment of coronary heart disease after PCI combined with psychological disorders(qi and blood stagnation syndrome),and the the experimental group is better than the control group.2.Starting from Qi and Blood,treating syndromes of coronary heart disease according to syndrome differentiation,benefiting qi and nourishing blood;dispersing stagnated liver qi for relieving qi stagnation,nourishing the heart to calm the mind to treating coronary heart disease after PCI combined with psychological disorders is effective.3.Yangxin Decoction addition and subtraction has clinical effect on coronary heart disease after PCI combined with psychological disorders is effective,without adverse reactions. |
