Objective:This study evaluated the characteristics of brain function in wilson disease patients with dysarthria by applying the Regional Homogeneity analysis of resting state functional magnetic resonance imaging,we aim to provide imaging assistance for its early diagnosis.Methods:?The Intra-Operative MRI System was used to collecte resting state BOLD signal data of 40 HLD patients and 15 normal healthy people by echo-planar imaging,and patients with dysarthria were screened by the Frenchay articulation assessment scale.After accepting the criteria for exclusion and elimination,15 patients remained Normal control group(NC),15 patients with HLD with speech impairment(HLD-SI),and 14 patients with HLD without speech impairment(HLD-NC).The local consistency(ReHo)analysis method was used to evaluate spontaneous brain activity changes in various brain regions of HLD patients with dysarthria in resting state.?An analysis of covariance(ANCOVA)was performed on the ReHo plots of the three groups of subjects,and then a Post-Hoc t test was performed between each group.The correlation between the ReHo values of the different brain regions of the HLD-SI group,the HLD-NC group,and the NC group and the Frenchay speech score were analyzed.Results:?The general data study of the three groups of subjects illustrated no significant difference in head movement parameters,gender,and age(P> 0.05);The HLD-SI group and HLD-NC group have significant statistical significance in comparing the TCM Syndrome Scale score with the modified GAS score(P <0.05),and there was no significant difference in the course of the disease between the two groups(P> 0.05).?Compared with the Normal Control group,the HLD-SI group has the right inferior temporal gyrus,,the right central anterior gyrus,the right central posterior gyrus,the bilateral anterior wedge lobes,the left orbital forehead gyrus,the right middle temporal gyrus,ReHo enhancement in the left middle occipital gyrus;in the bilateral lenticular nucleus,the left superior temporal gyrus,the left central posterior gyrus,the cerebellar vermicompartment,the medial and lateral cingulate brain gyrus,and the right temporal pole: superior temporal ReHo decreased on the right occipital gyrus.Compared with the HLD-NC group,the ReHo of the left hemisphere medial forehead increased in the HLD-SI group;the ReHo of the medial and lateral cingulate gyrus decreased in the left hemisphere.Compared with the NC group,HLD-NC has enhanced ReHo in the right lingual gyrus,right central central gyrus,right upper marginal gyrus,right top upper gyrus,left upper forehead gyrus,and left occipital gyrus;Temporal pole: superior temporal gyrus,left orbital temporal inferior gyrus,left superior temporal gyrus,left central posterior gyrus,medial superior frontal gyrus of left hemisphere,and right-sided bean-shaped pale bulb ReHo decreased.?There was a significant correlation between the modified Frenchay articulation rating scale(FDA)and the modified Wilson disease comprehensive rating scale(modified GAS)in the HLD with dysphonia group,r = 0.950,p <0.05.?The Pearson correlation analysis found that the FDA score was negatively correlated with the ReHo values of the left lenticular nucleus,the medial and lateral cingulate gyrus of the left hemisphere,and positively correlated with the ReHo values of the right lower temporal gyrus and the right middle temporal gyrus.Conclusions:Reduced brain activity in the left medial and lateral cingulate gyrus and bilateral lenticular nucleus in patients with HLD dysphonia is related to impairment of articulation function in HLD patients.Increased brain activity in some temporal and frontal lobe may be a function of speech Compensation.Resting state functional magnetic resonance imaging can detect abnormal changes in brain functional activity early in the progression of HLD,and provide a new brain imaging diagnosis method for early identification of HLD articulation disorders.
