Diabetes is recognized as a chronic disease threatening health in the world.Its morbidity and mortality are increasing.Although most patients can control blood sugar temporarily through drug treatment and diet,taking drugs for a long time has brought many adverse emotions and side effects to patients,seriously interfering with the quality of life(QOL).It has been proved that physical exercise has a good prevention and treatment effect on diabetic patients.At present,there are few reports about the effect of community exercise intervention on diabetes control,and the existing related researches only judge the remission of the disease by the fluctuation of physiological and biochemical indexes,and pay little attention to the quality of life of patients.Therefore,my research group designed a community-based group control experiment to explore the impact of the integration of physical and medical intervention on the quality of life and physical fitness level of diabetic patients,trying to prove that this intervention mode can improve the quality of life,improve the level of physical fitness,and increase social well-being of diabetic patients in the community,so as to provide for the promotion of sports prevention and rehabilitation of diabetes Referential suggestions.This paper adopts the methods of literature collection,experiment and data arrangement and analysis.Two cities were randomly selected from 17 cities in Shandong Province,and then two communities were randomly selected from each city(the selected communities were similar in economic level,cultural level and population,but far away).Subjects were recruited through community hospitals,and then the two communities were randomly divided into control group and experimental group.76 diabetic patients were selected from Donggang District of Rizhao City There were 38 subjects in the control group and 38 subjects in the experimental group.70 diabetes patients were selected in Laiwu City.There were 35 subjects in the control group and 35 subjects in the experimental group.The experimental group received community medical and physical integration intervention,twice a week strength training and four times aerobic exercise.The control group maintained a normal lifestyle,the intervention time was three months.Three months later,we filled in the health questionnaire and tested the fitness index and fasting blood glucose.Results:(1)quality of life index: according to the data analysis finally included in Rizhao,the diabetic patients in the experimental group and the control group have significant differences in physiological function,physical pain,general health status and social function(p<0.05),among which there is a highly significant difference in physical pain(p<0.01),and the improvement in the experimental group is significantly higher than that in the control group,but there is no difference in physiological function,energy,emotional function and mental health(p > 0.05).According to Rizhao intention-to-treat analysis,the diabetic patients in the test group and the control group only have significant difference in physical pain(p<0.05),while other dimensional indexes have no significant difference(P > 0.05).According to the final data analysis and intention-to-treat analysis of Laiwu,there was no significant difference between the experimental group and the control group in all dimensions of quality of life of diabetic patients(P > 0.05).Meta-analysis results of the data finally included in the two places show that the difference in physiological function indexes has statistical significance [MD=4.13,95% CI(0.11,8.16),P = 0.04];(2)physical fitness indexes: from the perspective of the final data included in the sunshine analysis,the indexes of the experimental group and the control group were statistically significant There were significant differences in chair standing and sitting test,dumbbell arm bending test and chair sitting posture body forward bending test in diabetic patients(P < 0.01),and the improvement in the experimental group was significantly higher than that in the control group,but there was no difference in the 8-foot standing and 6-minute walking test(P > 0.05);from the analysis of sunshine intention to treat,the diabetes in the experimental group and the control group were analyzed There were significant differences in chair standing and sitting test,dumbbell arm bending test and chair sitting posture body forward bending test(P< 0.01),in 6-minute walking test(P < 0.05),but not in 8-foot standing and walking test(P >0.05);from the final data analysis and intention to treat analysis of Laiwu,There was no significant difference between the experimental group and the control group in the evaluation indexes of physical fitness(P > 0.05);the analysis of the effect value and confidence interval of the indexes related to physical fitness in the two places showed that the heterogeneity was too large,so there was no need to combine the data;(3)fasting blood glucose index: from theperspective of data analysis and sunshine intention to treat analysis,the diabetic patients in the experimental group and the control group were There was significant difference in fasting blood glucose index(P > 0.05),compared with the average value of fasting blood glucose index before and after the intervention in the experimental group,there was a downward trend;from the final data analysis and intention treatment analysis of Laiwu,there was significant difference in fasting blood glucose index between the experimental group and the control group(P > 0.05),but the fasting blood glucose index before and after the intervention in the experimental group was significantly different The mean value of fasting blood glucose decreased in different degrees.The results of meta analysis showed that: compared with the normal life style,the difference of fasting blood glucose index in community was statistically significant [MD =-0.91,95% CI,(-1.54,-0.28),P = 0.005].The data of intention to treat analysis showed that the difference of fasting blood glucose index was statistically significant [MD =-0.69,95% CI,(-1.24,-0.15),P = 0.01].Conclusion: according to the results of meta-analysis,the community medical and physical integration model can effectively control the fasting blood glucose of diabetic patients.From the perspective of sunshine data,the community medical and physical integration model can improve the quality of life,physical fitness and fasting blood glucose of diabetic patients,but it is based on the data differences between sunshine and Laiwu and the results of intention to treat analysis,Some indicators have not improved significantly,so we think community-based sports intervention in sports prescription,instructor training,medical supervision and other aspects need to do further research and design,in order to better improve the quality of life of diabetic patients.
