Objective: to investigate the therapeutic effect of postoperative percutaneous acupuncture and acupoint application on preventing and curing nausea and vomiting(PONV)after knee replacement surgery,and whether there is interaction between the two.Method: Eighty high risk PONV patients who volunteered for knee arthroplasty were selected.The above patients were divided into 4 groups on average,namely,blank control group,acupoint application group,acupuncture group and acupuncture application group,with 20 cases in each group.In the perioperative period,patients in the blank control group were treated with routine postoperative fluid supplementation and acid inhibition,and other drugs and therapeutic intervention were not accepted.In the acupoint application group(traditional Chinese medicine evodia,ginger,cinnamon),postoperative application treatment and routine fluid supplementation were given.Within 20 minutes after the surgery,the patients were treated by the same physician with acupoint application therapy(neiguan point,liangmen point,zhongwan point),which was removed after 24 hours.Acupuncture group(0.3*40mm one inch and a half),Postoperative acupuncture treatment and routine fluid supplementation were given.The same physician performed acupuncture intervention on the patients within 30 minutes before surgery.The acupoints were selected from neiguan,hegu,quchi and zusanli points.The acupuncture group was treated with acupuncture point application and acupuncture at the same time.The occurrence of nausea and vomiting(PONV)within 24 hours after the operation was recorded;Nausea and vomiting were rated as 0 ~ 3 by visual simulation score of nausea degree and WHO standard;The time and duration of postoperative nausea and vomiting were clearly recorded;Analysis of factor variance was used to determine whether acupuncture and acupoint application had interactive effects.Results: 80 female patients after knee arthroplasty were included in the study.Comparing the baseline data of the four groups,including age,height,weight,operation time,intraoperative blood loss and infusion volume,The difference was not statistically significant(P > 0.05).Compared with the blank control group,the incidence of PONV in acupuncture group,paste group and acupuncture paste group was reduced(P< 0.05).;The PONV severity of the acupuncture group,the paste group and the acupuncture paste group decreased(P < 0.05),and the decrease of the acupuncture group was more obvious than that of the paste group,but there was no interaction between the two groups(P > 0.05);The duration of PONV in the acupuncture group,the paste group and the acupuncture paste group decreased(P < 0.05),and the duration of the acupuncture paste group was significantly lower than that in the acupuncture group and the paste group,there is an interaction between the two(P < 0.05)?Conclusion: Acupuncture,acupoint plaster and acupuncture point plaster combined treatment can reduce the incidence of PONV in patients after knee arthroplasty,reduce the severity of PONV,and shorten the duration of PONV.There was no interaction between acupuncture and plaster in reducing the severity of PONV,However,the two have obvious synergies in shortening the PONV duration.
