Objective: To explore the effect of nutrition intervention on rehabilitation and longterm prognosis of elderly patients with hip and knee arthroplasty.Methods: 85 elderly patients who underwent hip and knee arthroplasty in orthopedics department from September 2018 to September 2019 were randomly divided into experimental group and control group.43 patients in the experimental group received multidisciplinary diagnosis and treatment(MDT group).42 patients in the control group received routine treatment(non-MDT group).The basic conditions,nutrition,inflammation,biochemical indicators,hospitalization cost,hospitalization time,operation time,complications and intestinal function recovery were compared between the two groups.Results: 1.Baseline data: there was no significant difference between the two groups(P > 0.05),Nutritional status: the mean value of total protein in the MDT group was lower than that in the control group(nonMDT group)on the first day before operation,and the difference was statistically significant(P < 0.05).The mean value of total protein and albumin in the MDT group was higher than that in the non-MDT group on the seventh day after operation,and the difference was statistically significant(P < 0.05);the nrs2002 score in the MDT group was higher than that in the non-MDT group on the third day before and after operation,and the nrs2002 score in the MDT group was lower on the seventh day after operation In non-MDT group,the difference was statistically significant(P < 0.05).3.Comparison of inflammatory indexes: there was no significant difference between the two groups(P > 0.05)in the level of IL-6 and C-reactive protein before and 3 days after operation.The average level of IL-6 and Creactive protein in MDT group was lower than that in non-MDT group(P < 0.05)at 7 days after operation,and there was no significant difference in the level of procalcitonin between the two groups at different time points(P > 0.05).4.Biochemical indexes: there was no significant difference in alanine aminotransferase,glutamic oxaloacetylase and creatinine between the two groups before operation,3 days after operation and 7 days after operation(P > 0.05);the average value of urea nitrogen in MDT group was lower than that in nonMDT group,and the difference was statistically significant(P < 0.05).5.Index of Health Economics: the time of stay in ICU,operation time,hospitalization time and hospitalization expenses of MDT group were lower than those of MDT group,and the difference between hospitalization days and hospitalization expenses was statistically significant(P < 0.05).6.Others: the number of intestinal function restored to normal in MDT group was more than that in non-MDT group,with statistical significance(P < 0.05);there was no case of incision infection in MDT group,and there were 3 cases in non-MDT group,with no statistical significance(P > 0.05).Conclusion: The nutritional intervention under MDT mode in perioperative period can effectively promote the postoperative rehabilitation of elderly patients with fracture,reduce the incidence of infection and various complications,and accelerate the recovery of patients.
