Objectives1.To describe the status of dignity,social support and hope level in patients with middle and advanced cancer.2.To explore the influential factors of dignity in patients with middle and advanced cancer.3.To analyze the mediator effect of dignity between social support and hope level.To provide theoretical basis for the clinical implementation of targeted nursing measures.MethodThe Patient Dignity Inventory,the Edmonton Symptom Assessment Scale,the Social Support questionnaire and hoping level scale were utilized to investigate 425patients with middle and advanced cancer in Shandong province.SPSS22.0 was used for the data analysis.The research included statistical methods:descriptive statistics,t-test,one-way ANOVA,multiple stepwise regression analysis,Pearson correlation analysis and mediator effect analysis.Results1.The average score of the dignity in patients with middle and advanced cancer was(55.79±21.47),which reached the medium level;the average score of physical and mental pain was(16.08±6.13),independence was(12.43±5.64),self-cognitive was(14.04±5.88),social role was(7.16±3.30),social support was(6.08±2.88);200 patients total score less than 50,225 patients total score more than 50,the incidence rate of the loss of dignity in patients with middle and advanced cancer was 52.94%.2.The average score of social support in patients with middle and advanced cancer was(34.85±9.63),higher than that domestic norm(34.56±3.73),which arrived at the moderate level.3.The average score of hoping level in patients with middle and advancedcancer was(33.54±6.38),which got to the moderate level.4.The single factor variance analysis showed that there was statistical significance in the dignity of middle and advanced cancer patients with different gender,age,occupation,disease awareness,family income per capita,payment method,treatment method,metastasis and self-care of difference(p<0.05).5.The single factor variance analysis clarified that the dignity of patients with middle and advanced cancer was influenced by physical problems(pain,fatigue,nausea,loss of appetite,drowsiness,itching,shortness of breath)and psychological problems(anxiety,depression),and quality of life.6.Multiple stepwise regression analysis indicated that family income per capita,fatigue,treatment,pain,self-care,gender,shortness of breath,occupation status as predictors of dignity in patients with middle and advanced cancer,the regression coefficients were-9.714,3.056,2.350,2.520,3.470,1.845,1.057,the coefficient of determination R~2=0.557,the above factors can explain 55.7%of the variation of dignity.7.Pearson correlation analysis exhibited that the loss of dignity was negatively correlated with the social support(r=-0.320),the loss of dignity was negatively correlated with the level of hope(r=-0.569),and social support was positively correlated with the level of hope(r=0.515).The direct effect of social support on hoping level was 0.515,and the indirect effect of social support on the level of hope through the dignity of the intermediary was 0.144.Dignity played a part of the intermediary role in social support and hoping level.Conclusion1.The dignity,social support,hoping level of patient with advance cancer were at the moderate level."Total score 50 points or more"is defined as apparent loss of dignity,the incidence rate of the loss of dignity in patients with middle and advanced cancer was52.94%.2.The dignity of patients with middle and advanced cancer was influenced by gender,age,occupation,disease awareness,family income per capita,payment method,treatment method,metastasis,self-care,health problems(pain,fatigue,nausea,loss of appetite,drowsiness,itching,shortness of breath)and psychological problems(anxiety,depression),and quality of life.3.Dignity of patients with middle and advanced cancer was negative correlated with social support and hoping level.Dignity played a part of the intermediary role in social support and hoping level.
