ObjectiveThrough clinical research,discussion of diabetes patients with total knee replacement(TKA),The length of time hospital,hospital expenses perioperative blood loss,blood transfusion,postoperative infections,lower extremities thrombosis occurred after operation,preoperative and postoperative knee function score 1 year,predict possible knee arthroplasty patients with diabetic complications,formulate corresponding countermeasures and preventive measures in a timely manner.Methods1.Cases collection and grouping: according to the inclusion criteria,2014 in December 2014 to present,who have TKA surgery in our joint surgery,while patients with type 2 diabetes,patients with knee osteoarthritis,21 cases(21)(group A)and 37 cases(37)pure osteoarthritis patients(group B)clinical data.2.Methods: A group of patients after admission application of hypoglycemic drugs or insulin to fall blood glucose,the fasting glucose<7.8mmol/L and postprandial blood glucose2h<10.0mmol/L,perfecting the necessary examination has no obvious operation taboo before TKA surgery treatment,B group of patients with complete examination has no obvious operation necessary taboo before TKA surgery.3.Evaluation: monitor and record the postoperative blood glucose group A,we adjust the quantity of oral medications and insulin according to blood glucose condition timely,control blood glucose steady,recording the length of hospital stay,hospital expenses,two groups of patients perioperative blood loss,Postoperative blood transfusion,postoperative infections,lower extremities thrombosis occurred after operation,preoperative and postoperative knee function score 1 year data,applied statistics software to carry on the analysis,Comparison of diabetes to knee arthroplasty whether there is a variety of factors.ResultsTwo groups of patients have not seen the obvious statistical difference(P>0.05)in age,sex and body mass index basic situation.Patients perioperative blood loss,hospitalization costs,lower limb venous thrombosis occurs,preoperative and postoperative 1 years of HSS score,there is a clear difference between two groups(P<0.05),the two groups of patients did not see clear statistical difference(P>0.05)in length of hospital stay,postoperative blood transfusion and postoperative infection status.Patients in group A,11 cases of postoperative blood glucose,up to 10.8mmol/L on an empty stomach,postprandial blood sugar up to 15.2mmol/L,adjust the dose of drug and insulin according to the circumstance,stable blood sugar in all 4 days postoperatively,but compared with the preoperative blood glucose level is still A bit higher.Conclusion1.patients with diabetes have influence in different level to TKA surgery at hospitalization expenses,perioperative blood loss,postoperative complications,preopera tive and postoperative thrombosis of lower limb knee functional recovery 1 years fa ctors.2.There are no evidence found that Diabetes have apparent effect to TKA surgery patients at length of hospital stay,postoperative blood transfusion,postoperative incision infection.3.The postoperative knee arthroplasties merged diabetes blood sugar easily fluctuate,we shoud pay attention to the monitoring of blood sugar,and processed in a timely manner.
