Objective:1.To explore the clinicopathological features of ovarian endometriosis(OEM)and find out the related risk factors,so as to provide some theoretical basis for primary prevention of ovarian endometriosis,so as to reduce the risk of OEM.2.To analyze the difference and correlation between the related factors of mild ovarian endometriosis(OEM)mild group(stage I and II)and severe group(stage III and IV),so as to provide evidence for clinical treatment.3.To study the correlation between ectopic endometrium and in situ intima,and analyzed the periodic changes of ectopic endometrium and the original endometrium from morphological aspects,so as to lay a foundation for further exploring the mechanism of OEM.Methods:1.Using a case-control study method,during the period from December 2013 to December 2017 in Affiliated Hospital of Tai'an Medical College,pathologically confirmed 637 cases of OEM as case group,mean age 46.43 + 7.32 years;at the same time to collect female patients and 637 cases of non OEM ovarian benign tumor of the medical records as the control group,the average age was 44.23 + 8.19 years old.Take access to case information,telephone follow-up and bedside consultation,fill out the "Questionnaire" related factors of ovarian endometriosis according to the unified standard,including general history,menstrual and reproductive history,obstetrics and gynecology operation and other related diseases and 30 more detailed index.All data were processed by software package,database was established.Statistical analysis was performed by SPSS21.0 software,row difference test and single factor Logistic regression analysis.After that,the statistical significance indexes were included in multivariate Logistic regression analysis.2.The use of control-a case study method,according to the rAFS staging of 637 cases of OEM patients were divided into mild group(stage I and II)and severe group(III and IV);comparison of the two age,the degree of dysmenorrhea,pregnancy,disease duration,serum CA125 levels,statistical analysis using SPSS21.0 software.According to the data of the two groups were respectively studied the difference and correlation test.3.A optical microscope was used to observe the paraffin section of the patients with HE staining,and the morphological differences between the endometrium and the in situ endometrium were compared.Results:1.After the 2 test,Mann-Whitney U test and single factor Logistic regression analysis showed that pregnancy,abortion,abortion,occupation,BMI,by volume,dysmenorrhea,uterine cavity operation history,pelvic operation history,pelvic inflammation,uterine leiomyoma,adenomyosis,endometrial hyperplasia or polyps are factors the influence of OEM.Multivariate Logistic analysis showed that the independent risk factors of OEM were dysmenorrhea,endometrial hyperplasia or polyp,adenomyosis,abortion,and uterine leiomyoma.2.By the Spearman rank correlation analysis and difference test(Mann-Whitney U test or t test),OEM stage has significant correlation with the course of disease,CA125,pregnancy,dysmenorrhea,or group of severe ovarian endometriosis in the course of disease,CA125,pregnancy and dysmenorrhea were significantly higher than those of mild OEM group.3.From the perspective of histomorphology,the ovarian endometriosis is similar to the normal endometrium.The coincidence rate between ectopic and in situ endometrium is up to 95%,suggesting that a hormone may regulate ovarian ectopic glands and interstitial and endometrial intima simultaneously.Conclusions:1.Endometriosis or polyps,myoma of uterus,adenomyosis,abortion,and dysmenorrhea are independent risk factors for ovarian endometriosis(OEM).If the above-mentioned factors are intervened,it may provide guidance for the prevention and treatment of ovarian endometriosis(OEM).2.There is significant difference in dysmenorrhea,pregnancy history and serum CA125 level in the stage of ovarian endometriosis,and is positively correlated with the stage of ovarian endometriosis.It can be used to evaluate the severity of ovarian endometriosis.3.The hormones regulating the endometrium may regulate the ectopic endometrium on the ovary.
