BACKGROUNDSleep is the basic physiological need for human survival.However,in recent years,reports have been arising continually that Chinese medical students have different degrees of sleep quality problems.And the detection rate of sleep disorders was as high as 59.83%.These problems will not only affect the physical and mental health of medical students,but also reduce their learning efficiency and social adaptability.Cognitive behavioral intervention is the first line therapy of psychological intervention.But the traditional mode of execution,the face-to-face way,is difficult to widely develop affected by geographical,economic and other factors.With the development of the concept "Internet plus",the WeChat platform has become a necessary way for daily communication among college students.It conforms to the trend of The Times and meet the needs of the information age to use WeChat to proceed the cognitive behavioral intervention.The purpose of this study is to investigate the sleep quality of medical students,to explore the effect of WeChat-based cognitive behavior intervention on improving sleep quality,and supplement the existing sleep quality intervention methods.METHODSThis study is divided into two parts:Part 1 The investigation of sleep quality of medical studentsFrom September 25 to 26,2018,800 medical students from a medical college were enrolled through convenient sampling.We use the Pittsburgh Sleep Quality Index(PSQI)to investigate the status of sleep quality of medical students and to compare and analyze the sleep quality and influencing factors among different genders,grades and majors.Part 2 Study on sleep quality intervention of medical studentsThere were 88 medical students with total score of PSQI^8 invited,65 of them responded.After screening and eliminating the physiological and psychological factors that have been known to affect sleep quality,60 medieal students who were included in this study were randomly divided into control group and intervention group with 30 in each group.The control group was blank group,and the intervention group received cognitive behavioral intervention through wechat platform.At the end of the 4th week,PSQI was used to compare the effect of the intervention study.RESULTSSurvey results of sleep quality of medical students1.Scores of the medical students' sleep quality The total PSQI score of 800 medical students was(5.72±3.24)points.The detection rates of good sleep quality(PSQI total score?4),general sleep quality(5?PSQI total score?7)and sleep disorders(PSQI total score?8)are 41.75%?28.88%?29.38%respectively.The scores of 7 factors that constituted the total PSQI score are subjective sleep quality(1.11 ±0.73),fall asleep time(1.18± 1.05),sleep time(1.14±0.85),sleep efficiency(0.49±0.78),sleep disorders(1.06±0.54),hypnotic drugs(0.09±0.39)and daytime dysfunction(0.65±0.71).2.Sleep Quality Comparison of medical studentGender Female's PSQI total score and the scores of subjective sleep quality,fall asleep time,sleep disorders and daytime dysfunction are higher than males.The differences were statistically significant(t=2.531,-2.251,-3.167,-2.784,-2.789,all P<0.05).The detection rate of Sleep disorders in females was higher than that in males(?2=9.102,P<0.05).Grade The PSQI total scores and the scores of subjective sleep quality,fall asleep time,sleep time,sleep efficiency,sleep disorders,hypnotic drugs and daytime dysfunction of the third-year students were higher than those of the fireshmen and sophomores.The differences were statistically significant(F=43.916,5.202,66.681,5.359,20.064,6.593,9.575,29.046,all P<0.05).Major The PSQI total scores and the scores of subjective sleep quality,fall asleep time,sleep time,sleep disorder and daytime dysfunction of the medical students in different majors were compared.The differences were statistieally significant(F=5.912,3.336,5.979,3.592,5.163,2.968,all P<0.05).The PSOI total;scores of Nursing and clinical is the highest,and the students of Chinese medicine score lowest.There were statistieally significant differences in the detection rates of Sleep disorders among students from different majors(?2=42.920,P<0.05).The detection rates are higher in Nursing,midwifery and clinical specialty.Analysis on the influencing factors of sleep quality of medical studentThe sleep quality of medical students was positively correlated with grade and gender(P<0.01),they were the influencing factors of sleep quality of medical students(F=31.453,P<0.05,R2=0.103).Results of The Intervention Research1.There was no significant difference in the general data and PSQI scores between the two groups before intervention(P>0.05).After the intervention,except for 1 factor of hypnotic drugs,the total seore of PSQI and 6 factors including subjective sleep quality,time to fall asleep,sleep time,sleep efficiency,sleep disturbance and daytime dysfunction were significantly lower than scores before intervention(t=14.790,5.460,7.102,5.835,4.360,3.764,4.267,all P<0.05).In the control group,scores of hypnotic drug factors increased,and scores of PSQI total score,five factors including subjective sleep quality,fall asleep time,sleep time,sleep efficiency and sleep disturbance decreased compared with previous ones,with statistically significant differences(t=3.775,2.068,2.710,4.026,3.003,2.763,all P<0.05).2.The total score of PSQI,subjective sleep quality,sleep disorders and daytime dysfunction in the intervention group were sig1ificantly lower than those in the control group,with statistically significant differences(t=3.231,2.575,2.796,2.945,all P<0.05).CONCLUSION1.The sleep quality of medical students needs to be improved.29.38%of them have PSQI? 8.2.Grade and gender are the influencing factors of sleep quality of medical students,among which senior and female medical students have poor sleep quality.3.Cognitive behavior intervention based on WeChat platform can significantly improve the sleep quality of medical students.
