Objective: Sleep has an important role for the human body.As a special group of medical students,they have a widespread sleep quality problem due to the pressure of schoolwork,long academic period,long internship period and high industry risk.Quality of sleep problems will affect both physical and mental health of medical students,and also interfere with the normal study and work.In the industry,docters need to pay attention to his work for a long time,which requiring early intervention in sleep.The purpose of this study is to investigate the sleep quality and the related influencing factors of medical students,to evaluate the effects of Naikan cognitive therapy on sleep quality,to make positive efforts to improve the quality of sleep for medical students,as well as the promotion of major colleges and universities for reference.Methods: This paper is constituted for two parts.One study is through random cluster sampling to provide Pittsburgh Sleep Quality Index(PSQI),Adolescent Self-Rating Life Events Check(ASLEC),Chinese Perceived Stress Scale(CPSS)Family economic pressure,State-trait anxiety inventory(STAI).Personal Evaluation Questionnaire Analysis in Tianjin Medical University in April in 2017.Aim to investigate characteristics of sleep quality and factors related to the occurrence.The other study was used to screen the medical students who had sleep disorders which met the inclusion criteria and then randomly divided into intervention group and control group.The intervention group using Naikan cognitive therapy intervention,the control group without NCT intervention,other conditions are same to intervention group.Before and after the intervention,both two groups were evaluated with the scales described in study 1 to evaluate the intervention effect of the therapy on sleep quality.At the same time using the actrigraphy to measuring sleep quality every night to provide objective indicators.Results :(1)A total of 550 questionnaires were sent to investigate the quality of sleep in medical students.A total of 538 valid questionnaires were retrieved,of which 318 were women(59.10%)and 220 were male(40.90%),of which 144 were poor sleep quality students,Accounting for 26.7% of the total surveyed population;240 students in rural areas(44.60%),298 students in urban areas(55.40%),248 only child students(46.09%)and 290 non-only child students(53.91%).There were 38 single-parent families(7.06%)and 500 non-single-parent families(92.94%).The average age of all medical students surveyed was 20.42±1.92 years.(2)There was no significant difference in PSQI scale between different genders(P> 0.05).(3)Age was positively correlated with sleep quality,sleep efficiency,sleep disturbance,hypnotic drugs,daytime function,and total score.The results were statistically significant.(P< 0.01)There is no relationship between fall asleep time and sleep time and age.(4)There were no statistically significant differences in PSQI among medical students from different places of origin.(P> 0.05)The score of sleep quality in the only-child medical student was higher than that of the non-only medical student,and the difference was statistically significant.(P< 0.05)Single-parent medical students scored significantly lower in sleep time than non-primary medical students,and the difference was statistically significant.(P< 0.05)(5)The total PSQI scores and their respective scores were separately analyzed by Pearson correlation with the scores and factors of the Adolescent Life Events Scale.There was a overallpositive correlation between the two scales.(6)Pearson correlation analysis was conducted between the PSQI and the perception stress scale and trait anxiety scale,except hypnotic drug factors,the perception stress and trait anxiety scale were positively correlated with the PSQI factors(P< 0.01).(7)Pearson correlation analysis showed no correlation between fall asleep time,sleep time,sleep efficiency,sleep disorders,hypnotic drugs and family economic stress scale(p(29)0.05),and other factors were positively correlated with family economic stress scale(8)There were total of 70 people in the group conformed to inclusion criteria and exclusion criteria and after 7 days of NCT,the experimental group excluded 5 students while 2 in the control group.Eventually 63 people were complete the statistical analysis.(9)The intervention group in total score of PSQI and its sleep quality,sleep time,sleep efficiency,and also daytime function are statistically significant differences.However,there was no significant difference between the control group in the PSQI.The difference of the PSQI in the intervention group was lower than control group(10)On the ASLEC scale,after intervening with NCT,the scores of interpersonal relationship,learning pressure and punishment in intervention group were significantly lower than those before intervention,with significant difference,loss,and health factor was not significantly different from before intervention.The difference between the control group and before intervention was not significant.There were significant differences in scores of interpersonal relationship,study pressure,punishments and other factors after intervention between the two groups,and there was no significant difference between the two groups in the scores of health factors(11)After intervention,the scores of perception stress,trait anxiety in intervention group were significantly lower than those before intervention,the difference was significant.Control group pressure perception,trait anxiety retest scores compared with before the intervention was not significant.After the intervention between the two groups showed perception pressure,trait anxiety scores decreased significantly,the difference was statistically significant.(12)After the intervention,the intervention group's actigraphy data on total sleep time,deep sleep time,shallow sleep time were increased,the difference was significant.(p(27)0.01)The score difference between the actrigraphy of the intervention group was higher than that in control group.Conclusion: The characteristics of sleep quality in medical students showed more heavier than ordinary college students.The PSQI did not show gender differences.In this survey,we found that with the gradual increase of age,the quality of sleep of medical students gradually declined.This study found that sleep quality and adverse life events was significantly positively correlated,perceived Stress and sleep quality was significantly positively correlated,State-trait anxiety and sleep quality was significantly positively correlated,family economic pressure and sleep quality was significantly positively correlated.NCT can significantly improve the quality of sleep for medical students,both subjective and objective measuring showed this point.NCT intervention has a good effect on reducing medical students' perceived stress inrelationships,learning pressure,health,and other factors.At the same time,NCT intervention has a good effect on reducing medical students' stress perception and trait anxiety.On the issue of family economic pressure,after the intervention of NCT,the medical family's economic sense of stress has also significantly eased.
