Impact Of COPD With HP Infection On Pulmonary Function And Complications

Objective:To analyze the correlation between chronic obstructive pulmonary disease(COPD)and Helicobacter pylori(HP)infection on pulmonary function and the incidence of major complications,and to explore the influence of HP infection on the condition and quality of life of COPD patientsMethods:Collected COPD patients who visited the three wards of Respiratory Medicine Department of the Second Affiliated Hospital of Kunming Medical University from January to December 2014,and came to the hospital at least once a year for three consecutive years.They were tested for HP infection and divided into positive group(research group)60 cases and negative group(control group)54 cases according to HP test results.The following indicators were compared between the two groups:height,weight,age,smoking index,years of illness,pulmonary function(FVC,FVC%Pred,FEV1/FVC),FEV1,FEV1%Pred,FEV1 decline rate and the incidence of PaO2,PaCO2)and major complications(pulmonary heart disease,coronary heart disease,hypertension,diabetes,arrhythmia).The correlation between the above indicators and HP infection was evaluated using SPSS 19.0 Statistical software for statistical analysis.Results:1 There was no significant difference in age between the two groups(t=0.645,P=0.406),and there was no significant difference in sex ratio between the two groups by Chi-square test.There was no significant difference in height and weight between the two groups by t test of two independent samples(P>0.05);there was no significant difference in the number of years of illness between the two groups by Wilcoxon W rank sum test(P>0.05);there was no significant difference in smoking index between the two groups by Wilcoxon W rank sum test(P>0.05).It can be considered·that the two groups of patients are comparable.2 Two groups of patients were divided into mild,moderate,severe and extremely severe according to the value of FEV1%Pred,mild:FEV1%Pred(>80%),moderate:50%less than FEV1%Pred(<80%),severe:30%less than FEV1%Pred(<50%)and extreme:FEV1%Pred(<30%).In HP negative group,there were 12 mild cases(22.22%),17 moderate cases(31.48%),16 severe cases(29.63%)and 9 severe cases(16.67%).In HP positive group,11 cases were mild(1 8.33%),16 cases were moderate(26.67%),18 cases were severe(30.00%)and 15 cases were extremely severe(25.00%).Chi-square test showed that there was no significant difference in FEV1%Pred classification between the two groups(P>0.05).3 The lung function(FEV1,FEV1%Pred,FVC,FVC%Pred,FEV1/FVC%,Pa02,PaC02),right atrial diameter,pulmonary artery pressure and other indicators of the two groups in 2014 were tested by two independent samples t test,which indicated that there was significant difference in Pa02 between the two groups(P<0.05);the other indicators had no statistical significance(P>0.05).4 The incidence of major complications(pulmonary heart disease,coronary heart disease,hypertension,diabetes mellitus,arrhythmia)in the two groups was tested by chi-square test.The results showed that there was no significant difference in the incidence of complications between the two groups(P>0.05).5 The FEV1 values of the two groups in 2014,2015,2016 and 2017 were compared by independent sample t test.The results showed that there was no significant difference in FEV1 values between the two groups in each year.Spearman rank correlation analysis was used to analyze the changes of FEV1 in the two groups with different years.The results showed that there was no significant correlation between FEV1 and annual changes in the two groups(r=-0.141,P=0.070 and r=-0.104,P=0.1 04).The results showed that the correlation of FEV1 with years was significant(r=-0.126,P=0.026),that is to say,the change of FEV1 in COPD patients was negatively correlated with the increase of years,suggesting that FEV1 decreased with the increase of years.6 Based on the FEV1 values of the two groups in 2014,the difference of FEV1 changes between the two groups was calculated and compared with that in 2015,2016 and 2017 respectively.The difference of FEV1 changes between the two groups was compared by independent sample t test.The results showed that there was no significant difference in FEV1 changes between the two groups in each year.Spearman rank correlation analysis was used to analyze the change of FEV1 differences between the two groups with the year.The difference of FEV1 between the two groups had statistical significance(r=0.294,P=0.002 and R=0.244,P=0.022)with the increase of years.That is to say,with the increase of years,the difference of FEV1 between the two groups showed an upward trend.Conclusion(s):1 The results of this study showed that the decrease of Pa02 was more obvious in COPD patients with HP infection,but the change of PaC02 was not significantly different.2 The results of this study showed that there was no significant difference in the changes of right atria]diameter,pulmonary artery pressure and the incidence of major complications between COPD patients with or without HP infection.3 The results of this study showed that the lung function,especially FEV1,of COPD patients decreased with the increase of years.During the observation period of 3 years,there was no significant difrerence in the decline rate of FEV1 between COPD patients with or without HP infection.
