Clinical Observation And MiRNA-21 Expression Of Yishen Paizhuo Decoction On The Chronic Kidney Disease With Kidney Deficiency And Damp Tubidity

Objective:To observe the clinical effect of Yishen Paizhuo Recipe on stage 3-5 of chronic kidney disease and its effect on the expression of serum TGF-?1 and miRNA-21,and to evaluate the curative effect of Yishen Paizhuo Recipe on patients with stage 3-5 of chronic kidney disease And safety,speculatively observe the changes of TGF-?1 and miRNA-21 in the progression of chronic kidney disease.Methods:101 patients with stage 3-5 chronic kidney disease who were diagnosed in Nanjing Traditional Chinese Medicine Hospital from September 2017 to December 2019 and met the inclusion criteria of this study were selected.All patients met the diagnosti c criteria of the KDIGO-CKD Guidelines issued by the International Kidney Disease Org anization in 2012,and were divided into a control group and a treatment group accordin g to the random principle.There were 53 cases in the control group and 48 cases in the treatment group.The control group consisted of 24 males and 29 females with an avera ge age of(67.7±6.32)years;the treatment group had 26 males and 22 females with an average age of(68.6±5.78)years.The control group is dominated by Western medicine t reatment.According to the specific conditions of each patient,individualized treatment p1 ans are formulated,and symptomatic treatments such as low-salt,low-fat,high-quality,an d low-protein diet are given to control blood pressure,blood sugar,and blood lipids.Th e treatment group is supplemented with Yishen Paizhuo Recipe on the basis of the contr of group.The total course of treatment is 3 months.Before and after treatment,the two groups were tested for blood,urine routine,liver and kidney function,electrolytes,TGF-?1,miRNA-21,and observed and recorded the relevant.clinical symptoms,urea nitrogen(BUN),blood creatinine(SCr),cyst Inhibin C(CysC),glomerular filtration rate(eGFR),changes in TGF-?1,miRNA-21,calculate the symptom score,and evaluate the clinical e fficacy of the two groups of patients.Results:miRNA-21 was positively correlated with BUN,SCr,CysC and TGF-?1,an d negatively correlated with eGFR,the difference was statistically significant(P<0.05),an d the correlation with TGF-?1 was better than the renal function index;two After treatm ent,the levels of BUN,serum creatinine and eGFR in the treatment group were better t han those in the control group,the difference was statistically significant(P<0.05);after treatment,the expression levels of CysC,TGF-?1 and miRNA-21 in the treatment group decreased compared with the control group Obviously,the difference was statistically sign ificant(P<0.05);the total effective rate of clinical disease efficacy in the treatment group(93.8%)was significantly better than that in the control group(75.5%),and the differen ce was statistically significant(P<0.05).Conclusion:1.Yishen Paizhuo Decoction can effectively reduce BUN and SCr in C KD patients,improve glomerular filtration rate,improve clinical symptoms,protect renal function,and effectively delay the progress of CKD.The expression of miRNA-21 and T GF-?1 has a significant down-regulation effect.It is speculated that the anti-fibrotic effec t may be exerted through the regulatory mechanism of miRNA-21,thereby improving the prognosis of CKD patients.3.miRNA-21 may be considered as an assessment of renal fibrosis Prognostic indicators.
