Clinical Observation On Yishen Paizhuo Decoction In Treatmenting Chronic Kidney Disease And Its Effect On FGF23 And Klotho

Objective:Observation on the efficacy of Yisen Paizhuo Decoction on chronic kidney disease with syndrome of kidney deficiency and dampness,and the influence on FGF23 and Klotho.Eevaluate the efficacy and safety of Yishen Pazhuo Decoction in the treatment of Chronic Kidney Disease.Predict the value of FGF23 and Klotho in the progress of Chronic Kidney Disease.Methods:Select patients with Chronic Kidney Disease who meet the inclusion criteria of this study,78 cases in total.According to the random principlethey were divided into treatment group and control group,38 patients in the treatment group and 40 patients in the control group.The control group developed an individualized treatment programs based on the actual condition of the patient.Strictly control the patient's dietary intake,blood pressure,blood sugar,and electrolytes.The treatment group was treated with Yishen Paizhuo Decoction in the basic treatment of the control group.Before and after treatment in both groups of patients.check blood routine,urine routine,liver and kidney function,iPTH,FGF23,Klotho,and record the changes of clinical symptoms,blood calcium,blood phosphorus,iPTH,BUN,Ser;eGFR,FGF23 and Klotho before and after treatment with a course of 3 months.Calculate symptom scores,and evaluate the clinical efficacy and TCM syndrome efficacy of the two groups.Results:FGF23 was positively correlated with Scr,blood phosphorus and iPTH,negatively correlated with eGFR and blood calcium,Klotho was negatively correlated with Scr,blood phosphorus and iPTH,and positively correlated with eGFR and blood calcium,all of them have significant correlation(P<005).The total efficiency rate of treatment group was 97.4%and control group was 72.5%,the treatment group was better than the control group.In the TCM syndrome integral,two groups all could i mprove and have statistically significant difference(P<0.001),and the compansion between groups after the treatment showed that the treatment group was better efficacy than the control group.The blood phosphorus,i PTH,BUN,Scr and eGFR in the treatment group were significantly lower than those in the control group(P<0.05).The level of serum calcium treatment was statistically significant compared with before treatment,but there was no significant difference between the treatment group and the control ogroup after treatment(P>0.05).After treatment,the level of FGF23.Klotho was decreased significantly in the treatment group(P<0.05 or P<0.001Conclusion:1.Yishen Paizhuo Decoction can relieve the clinical symptoms of patients with chronic kidney disease effectively,reduce the degree of BUN and Scr,and improve eGFR,protect patients' residual renal function and delay the progression of disease.2.The treatment group has a better,improvement in renal function than the control group,which can effectively alleviate the disorder of calcium and phosphorus metabolism in patients with chronic kidney disease.3.FGF23 and Klotho may improve renal function by regulating calcium and phosphorus,them can be considered as a prognostic factor for evaluating the progression of CKD.4.Yishen Paizhuo Decoction can impro FGF23 and Klotho of patients with Chronic Kidney Disease effectively to regulate the disturbance of calcium and phosphorus in CKD.
