Objective: In this study,22 children with simple adenoid hypertrophy and adenoidectomy and 26 normal children were studied by cephalometric analysis and comparison of lateral cephalometric films before and after adenoidectomy in children with adenoid hypertrophy and normal children,to study the effect of adenoidectomy on the development of maxillofacial bone in children,so as to provide theoretical basis for early orthodontic treatment.Method: The clinical data of 22 patients with simple adenoid hypertrophy and adenoidectomy in the Otorhinolaryngology Department of the first affiliated Hospital of Dalian Medical University from October 2017 to October 2019 was retrospectively analyzed, including 16 males and 6 females, with an average age of 5.6 ±3.1 years. The control group was defined as group B.26 children aged 13 years old who were treated in the Department of Pediatrics or Otorhinolaryngology of the first affiliated Hospital of Dalian Medical University were selected. They have no symptoms such as adenoid tonsillar hypertrophy and upper airway obstruction, including 15 males and 11 females, with an average age of 6.8 ±2.5 years. The patients in the experimental group were followed up for half a year and were divided into preoperative group and postoperative group, which were defined as group A1 and group A2 respectively. The patients in the three groups were examined by lateral cephalography, and the lateral cephalograms of all subjects were imported into the computer.Using Winceph8.0 cephalometric software,10 cephalometric indexes representing maxillofacial development were selected: SNA,SNB,NP-FH,NA-PA,U IL ?,U I-SN,L I-MP,MP-FH,MP-SN,Y-axis angle. The lateral cephalograms of all subjects were traced and measured by the same technician. All the measured data was statistically analyzed by SPSS22.0 software, among which group A1,group A2 and group B were analyzed by t-test of mean comparison(the kurtosis and skewness of each group were analyzed by statistical software, and the variance was homogeneous),and there was statistical difference between group A1,group A2 and group B(P<0.05).The data of each group was analyzed by statistical software, and the kurtosis and skewness were in accordance with normal distribution. Results: 1.There was no statistical difference in age and sex between the experimental group and the control group.2.According to the statistical analysis of the cephalometric data of the preoperative group and the control group, the kurtosis and skewness of the two groups basically accorded with the normal distribution, the variance was uniform, and two independent samples t-test can be used. The results showed that there were significant differences in NA-PA,MP-FH,Y-axis angle and U?-SN between group A1 and group B(P<0.05).The angle of NA-PA,MP-FH and Y-axis in group A1 was larger than that in group B,and the angle of U I-SN in group A1 was smaller than that in group B.There was no statistical difference among the other groups.3.According to the statistical analysis of the cephalometric data of the postoperative group and the control group, the kurtosis and skewness of the two groups basically conformed to the normal distribution and the variance was uniform, and the data between the two groups was tested by two independent samples t-test.The results showed that there was only statistical difference in Yaxis angle between the two groups, and there was no statistical difference in other measurement indexes.4.The cephalometric data of preoperative group and postoperative group was analyzed by two paired samples t-test.The results showed that there were significant differences in NA-PA,MP-FH,U?-L? and U?-SN between A1 group and A2 group(P<0.05).Compared with A1 group,NA-PA,U?-L? and MP-FH were smaller and U?-SN was larger in A2 group. There was no significant difference among other groups. Conclusion: 1.Adenoid hypertrophy affects the facial development of children's teeth and jaws, and causes bone-like misjaw malformation in children.2.After adenoidectomy, the patient can not only improve the symptoms of upper airway obstruction, but also affect the patient's jaw facial development.3.Although abnormal facial development in the jaws of patients with adenoidectomy has improved,it is difficult to fully return to normal cynome.
