| Objective:To observe and appraise the clinical curative efficacy of Chaiqin Qingying Decoction in treating Subacute Thyroiditis(the type of stagnated fire and phlegm obstruction),and to evaluate new treatment ways and experiences of Subacute Thyroiditis with the therapy of traditional of Chinese medicine.Methods:This subject collected 72 patients with Subacute Thyroiditis(the type of stagnated fire and phlegm obstruction)who were adopted to the department of endocrinology of the first affiliated hospital of Yunnan university of traditional Chinese medicine from December 2018 to December 2019.They were randomly divided into control group and treatment group,36 cases were included in the control group that was treated with the western medicine Prednisone,and 36 cases were included in the treatment group that was added the traditional Chinese medicine Chaiqin Qingying Decoction on the basis of the western drug prednisone for 8 weeks.After drug withdrawal,the changes of TCM syndromes,serum thyroid hormones,erythrocyte sedimentation rate(ESR)before and after treatment as well as the evaluation of hormone dosages,the total effective rate and 4 weeks after discontinuation of relapse were statistically analyzed.Results:1.During the study,3 cases fell off and 33 cases were included in the treatment group,2 cases were fell off and 32 cases were included in the control group due todisconnection and poor patient compliance.2.TCM syndromes:it showed that the TCM total score in the treatment group was significantly different from that in the control group in terms of the relief of the clinical symptoms such as goiter,thyroid pain,fever,polydipsia,irritability,fatigue,palpitations and hyperhidrosis(P<0.01)after 8 weeks treatment.In terms of the single clinical symptom,symptoms of goiter,and thyroid pain,polydipsia,hyperhidrosis,fatigue,palpitations and irritability in the treatment group were significantly better than that in the control group(P<0.01),and symptoms fever and tremor were no statistical significance(P>0.05)compared between the 2 groups.It shows that the application of Chinese medicine on the basis of hormones could significantly improve the clinical symptoms more effectively than the simple application of hormones.3.In the evaluation of clinical efficacy,there were significant difference,indicating that Chinese medicine combined with hormone therapy can effectively improve the treatment efficiency.4.In terms of the serum index,there were significant differences both the two groups before and after 8 weeks of treatment(P<0.01),but there was no significant difference(P>0.05)between the these two groups after 8 weeks of treatment.It showed the changes of ESR and FT3,FT4,TSH were similar between the two groups.5.Hormone dosage analysis: the average dose of hormone in the treatment group was less than the average dose of hormone in the control group indicated that the dosage of hormone combined with traditional Chinese medicine was significantly lower than that of hormone alone.6.In terms of recurrence rate and side effects:6.1 percent of the treatment group recrudesced and 25 percent of the control group recrudesced after4 weeks of discontinuation with statistically significant difference(P<0.05),showing that the treatment group could reduce the recurrence rate after drug withdrawal.There were no obvious adverse reactions in the two groups during the observation of this research.Conclusion:Chaiqin Qingying Decoction combined with prednisone can significantly improve therelevant TCM syndromes of Subacute Thyroiditis(type of stagnated fire and phlegm obstruction),and can effectively reduce the dosage of prednisone,reducing the side effects of hormone use to bring a large amount for a long time,increased patient compliance.At the same time,it can reduce ESR and the recurrence rate which be worth of clinical promotion. |
