Objective:Through the collection and statistical analysis of domestic literature on acupuncture analgesia mechanism in the past half century,the research results and future research directions on acupuncture analgesia mechanism are summarized,and the dominant diseases of acupuncture analgesia and related influencing factors in clinical application are summarized,which provides the literature basis for the future research on acupuncture analgesia.Methods:By computer retrieval of the CNKI?VIP and Wan fang data base,according to the inclusion of exclusion criteria input documents,using Excel database for collation and statistical analysis.Results:1.The literature study of acupuncture analgesia mechanism is mainly divided into nervous system and connective tissue system according to system classification;876 articles of nerve mechanism and 363 articles of connective tissue according to inclusion conditions;128 articles of peripheral nervous system,287 articles of central nervous system and 461 articles of neurotransmitter type.2.The organization and structure of acupuncture analgesia:(1)acupuncture stimulates peripheral afferent fibers to participate in analgesia,and the finer the excitatory fibers,the greater the stimulation intensity,the stronger the analgesic effect;when the acupuncture signal is uploaded,it affects the activity of the skin and viscera dominated by the adjacent segments and the pain afferent of the adjacent segments through the segmental connections in the spinal cord,so as to play an analgesic role.(2)The centralmodulation pathway of acupuncture analgesia includes three parts: one is the descending inhibition pathway composed of the periaqueductal gray matter,the raphe nucleus and the dorsal horn of the spinal cord;the other is the negative feedback pathway composed of the spinal cord,the central hypothalamic nucleus,the ventrolateral orbital cortex,the periaqueductal gray matter and the spinal cord;the third is the cortical thalamic loop.(3)as a huge liquid crystal system in human body,the connective tissue can release bioelectricity through the coupling effect,that is,the mechanical stimulation liquid crystal of acupuncture releases bioelectricity through energy exchange,resulting in piezoelectric and anti piezoelectric effects,rapidly changing the calcium channel of the cells in the lesion,relieving the spasmodic and stiff muscles,and participating in acupuncture analgesia.3.The neurotransmitters of acupuncture analgesia mainly include endogenous opioid peptide(endorphin,enkephalin,dynorphin),5-hydroxytryptamine,noradrenaline,acetylcholine,substance P,?-aminobutyric acid,etc.,in which the content of endorphin,enkephalin,5-hydroxytryptamine,substance P,acetylcholine in the brain is positively related to acupuncture analgesia,substance P is negatively related to the spinal cord;dynorphin in the spinal cord It is involved in analgesia but not in the brain;noradrenaline and GABA are negatively correlated in the brain and positively correlated in the spinal cord when acupuncture;the research on neurotransmitters involved in acupuncture analgesia is mostly a single research,but the research on the synergistic effect of various neurotransmitters is still less.4.High frequency of acupuncture analgesia includes cervical spondylosis,arthritis,lumbago,headache,scapulohumeral periarthritis,migraine,dysmenorrhea,sciatica,trigeminal neuralgia,postherpetic neuralgia,epicondylitis,toothache,stomachache,etc.;the high frequency acupointprescription of acupuncture analgesia for each pain is mainly the acupoint around the pain site;the top ten acupuncture analgesia The high-frequency acupoints are Cervical Jiaji point(EX-B2),Ashi,Fengchi(GB20),Lumbar Jiaji point(EX-B2),Da changshu(BL25),Houxi(SI3),Yanglingquan(GB34),Tianzhu(BL10),Dazhui(DU14)and Taiyang(EX-HN5).Conclusion:1.The study of acupuncture analgesia mechanism is based on the anatomy of nerve structure: from the peripheral nerve fiber spinal cord subcortical cerebral cortex,and its related neurotransmitters on this pathway as the leading factor,it constitutes the study of acupuncture analgesia with multi-level structure and multi neurotransmitters,and the characteristics of acupuncture analgesia with Shenjing mechanism as the leading factor.However,there are few researches on the synergistic effect of various neurotransmitters in acupuncture analgesia,which need to be further explored.2.The non neural mechanism of acupuncture analgesia is mainly the mechanism of connective tissue.Through the coupling effect,bioelectricity can be released to rapidly change the calcium channel of the cells in the lesion,so that the spasmodic and stiff muscles can be relieved and participate in acupuncture analgesia.However,compared with the neural mechanism,it is the relationship between the whole and the part.The research of connective tissue as the pain producing and pain relieving part is still in the initial stage.The mechanism of the relationship between connective tissue and the pain relieving mechanism of the nervous system is not clear,which needs further research and confirmation.3.Acupuncture analgesia has a wide range of clinical applications,including the treatment of common clinical pain,mainly for the treatment of neuropathic pain and inflammatory pain.The high frequency acupoints of acupuncture analgesia are mainly different according to different painsymptoms,and the acupoints selected for acupuncture analgesia are mainly the proximal ones.
