Objective :Using intravenous methylprednisolone as a control,to explore the efficacy and safety of nebulized inhaled budesonide in the treatment of patients with acute exacerbation of chronic obstructive pulmonary disease incorporate with type 2 diabetes mellitus.Methods : To study the inpatients department of Respiratory Medicine of Shaoyang Central Hospital from November 1,2017 to May 1,2019,160 patients with acute exacerbation of chronic obstructive pulmonary disease incorporate with type 2 diabetes mellitus.Randomly divided into two groups,78 of whom were given intravenous injection of methylprednisolone 40 mg everyday for the control group,another 82 patients nebulized inhaled budesonide 6mg everyday for the observation group.The rest of the treatment and care of the two groups of patients were carried out in accordance with the conventional treatment of acute exacerbation of chronic obstructive pulmonary disease and type 2diabetes.The lung ventilation function and glucose metabolism indexes before treatment and after 1 week of treatment were tested.Results : 1.There was no significant difference in gender,age,height,weight,smoking index severity of illness including related indexes of lung ventilation function and glucose metabolism between the two groups before treatment(P > 0.05).2.The lung ventilation function indexes of the control group and the observation group were significantly improved after treatment.The difference was statistically significant(P<0.05).There was no significant difference between the two groups after treatment(P>0.05).3.Glucose metabolism indexes in the control group were significantly higher than before treatment,and the difference was statistically significant(P<0.05).Glucose metabolism indexes in the observation group after treatment were not significantly changed compared with before treatment,and the difference was not statistically significant(P>0.05).After treatment,the two groups were compared,the glucose metabolism index of the observation group was significantly lower than that of the control group,the difference was statistically significant(P<0.05).Conclusion:Nebulized inhalation of budesonide in the treatment of patients with acute exacerbation of chronic obstructive pulmonary disease incorporate with type 2 diabetes mellitus can improve pulmonary ventilation function.The efficacy is similar to that of intravenous methylprednisolone,but budesonide has little effect on glucose metabolism.
